To coordinate all key PCN patient activity, including improving access to services information and ensuring health and care planning is timely, efficient, and patient-centred.

To provide general assistance to the PCN and practice teams bringing together all the information about a patient's care and support needs to improve patient care.


Key Responsibilities:
To have a thorough knowledge of all Practice procedures and protocols, working within them all times.

Work with people, their families and carers to improve their understanding of the patients condition and support them to develop and review personalised care and support plans to manage their needs and achieve better healthcare outcomes.


Help people to manage their needs through answering queries, making and managing appointments, and ensuring that people have good quality written or verbal information to help them make choices about their care, using tools to understand peoples level of knowledge, confidence in skills in managing their own health.

Provide coordination and navigation for people and their carers across health and care services, working with other primary care professionals; helping to ensure patients receive a joined-up service and the most appropriate support.

To provide services to patients via clinics in the PCN, domiciliary visits and in residential and nursing homes if required, and to deliver care plans that improve the quality of patient care.

To present at patient group meetings or other appropriate events to give advice.


Work with our pharmacy teams and clinical leads to improve the care of certain cohorts of patients, as targeted to be in need by those who commission the care we provide or identified by ourselves for example following up patients with previous high blood pressure readings or arranging monitoring blood test appointments for patients on certain medication.

Working with lists of patients as identified via searches on the clinical system to issue invitations for appointments where appropriate, via telephone, text invitation and written form where required Following up patients who do not respond to initial invitations to appointments To help plan, develop and support the introduction of new working processes within the PCN to optimise the patient uptake.

Undertake any other additional duties appropriate to the post as requested by the PCN Clinical Director or the Primary Care Manager.

Working with lists of patients as identified via searches on the clinical system to issue invitations for appointments where appropriate, via telephone, text invitation and written form where required.


Other Duties Attend suitable courses as directed by the practice or network to enhance your skills either on site or external Attend all monthly training events as directed by the Practice/PCN Manager Demonstrate duties to new staff or less experienced staff Use own initiative and resourcefulness to resolve problems which occur as part of day to day duties.

Locating and providing appropriate patient documentation and helping to resolve minor problems.
