Chippenham Community Team

30 Hours per week - Permanent

If you love working in a Community setting wehave a great opportunity for a qualified Nursing Associate (Band 4) to joinChippenham Community Team. This post is currently 30 hours.

The team is based in Chippenham CommunityHospital and provides patient care between hours with shiftpatterns including some weekends, early and late shifts which can be flexible.The team supports patients who are registered with GP practices in theChippenham locality, and we work closelythe wider Multi-Disciplinary team including our Therapists, Social Care,GPs, and other agencies.

This role is very rewarding and will includesupporting a caseload of patients and providing effective nursing interventionsin patients homes and other community settings. Our caseload includes patientswith palliative care needs, simple and complex tissue viability needs and arange of acute and long-term conditions. You will be required to have or besupported towards achieving a wide range of nursing competencies includingmedication administration, patient assessments, wound management, andcontinence care.

Main duties of the job

You will also need some experience in NHS patient care, alongsideexcellent interpersonal and communication skills. We are a busy, cohesive andwell-organised team and will welcome a new member of staff who is enthusiastic,resilient and flexible and also willing to contribute to the ongoingdevelopment of the services we provide.

Some experience of working within a health and social care environment Ability to communicate with members of the public and health and care providers Courteous, respectful and helpful at all times Ability to deal with non-routine and unpredictable nature of the workload and individual patient contact Knowledge of when to seek advice and refer to a registered care professional

Desirable

Ability to work on own initiative Ability to take part in reflective practice and clinical supervision activities Understanding of evidence based practice Understanding of basic physiology, normal vital signs, fluid balance, nutritional requirements etc.
