Southern Health Foundation Trust has an exciting opportunity for a Band 3 Healthcare Support Worker to join the Urgent Community Response Team and Community Assessment Unit. The post is a full--time or part-time substantive post covering seven days per week on a variety of hours.

Working in the community you will be providing care to housebound patients with complex care needs, enabling patients to receive the care they need in their own home.

This is an excellent opportunity if you are thinking of joining an Urgent Community Response team in the community or broadening and gaining new experiences. This role is critical to preventing hospital admissions as part of the Ageing Well agenda, and monitoring patients in their own home as part of the developing virtual ward. The role will be delivered both in patient's own homes and clinic -based care within a Community Assessment Unit. We are looking for an enthusiastic, caring & motivated professional with excellent clinical & communication skills, who can work autonomously, supporting the team lead to manage the case load and staff development.

The post holders must have relevant experience in health care and demonstrate a proactive and positive attitude to learning the skills required to fulfill the role.

Access to a vehicle and possession of a full driving license is essential.

Main duties of the job

The Health Care Support Worker will have responsibility for participating in the delivery of patient care under the supervision of the Community Assessment Unit and Urgent Community Response Team. Following prescribed care plans and treatment protocols. The post holder will also provide general administration support to the team.

Work under supervision following written care plans and care pathways devised by the Registered Nurse.

Follow SHFT Policies and Procedures.

Has knowledge and skills to carry out prescribed tasks and will report to Registered Nurse any variances.

Our new Trust will continue to cater to the unique needs of different communities, making healthcare across the county more accessible, as well as offering staff more opportunities for career development, training and partnership working.
