_Are you a positive individual who is enthusiastic about providing support to young individuals with early onset dementia?_


Our service is a partnership between Community Links and Carers Leeds and offers a range of wellbeing, holistic, mutual and peer support, connecting people to groups and activities within their community helping people to live well with dementia.

You'll conduct activities which encourage independence as well as maintaining other key-worker responsibilities.

You'll support person-centred activities that promote independence and well-being on an individual and group basis and act as a keyworker to several clients.

**If you have: - Creative abilities towards working with vulnerable groups

  • Experience of developing support plans
  • Good verbal, written and numeracy skills
  • The ability to make strong connections
