CARE FACILITATOR – Long Term Living at Hereford

ORGANISATION RESPONSIBILITIES:

Reports to:
Registered Manager, Head of Services

MAIN PURPOSE OF JOB: To support the Registered Manager in running the residence successfully, ensuring the highest standards of care and support for residents. Also, promoting the health and well-being of residents at all times.


This role involves supervising and leading staff to encourage residents' independence, autonomy, community participation, and individual aspirations, following a holistic approach to individual care.


PRIMARY TASKS:
  • Ensure adequate staffing and efficient team operation while meeting resident requirements within budget constraints.
  • Set high standards of individualized care, enabling each person to enhance their independence and reach their full potential.
  • Administer medication and perform associated duties following policies and procedures.
  • Effectively communicate with relatives and professionals, ensuring facilitators also maintain good communication.
  • Participate in support and development programs, contributing to self and staff appraisals.
  • Maintain accurate documentation, including care plans and risk assessments, according to policies.
  • Collaborate with team members and report significant events to relevant team workers and managers.
  • Engage residents in community activities and promote community involvement within the residence.
  • Adhere to health and safety policies, procedures, and guidelines of the organization.
  • Undertake regular training and professional development as required.
  • Demonstrate commitment to equality and diversity through personal example and actions.
  • Perform other duties as assigned by superiors.

*HOURS OF WORK*
37 hours per week, on a rotating schedule based on resident needs, including weekends.

Job Type: Full-time


Pay: £11.91-£14.88 per hour


Expected hours: Up to 40 per week
