The post holder will be responsible for providing and receiving complex and sensitive information relating to patients with life limiting illnesses.


They will be responsible for providing efficient & effective administrative and clerical support service to the MacMillan Palliative Care Team, inputting audit data, assisting with analysis, and, ensuring smooth running of the service within the Trust.

Will manage the day-to-day co-ordination of team referrals. Managing and coordinating the Palliative care MDT meetings and dissemination of outcomes.

Whilst the post holder will be expected to be able to work autonomously, they will be closely supported and supervised by the CNS of that team.

The information and nature of the communication required is sensitive due to the nature of palliative care. Communication in this context requires a high degree of empathy, understanding, diplomacy, honesty and integrity.

This vacancy is advertised on a fixed term basis for a period of 6 months (or on the return of the substantive postholder) as it has been established to provide cover for an employee on maternity leave

  • Prioritise own workload as necessary. Identification of urgent referrals and highlighting to the appropriate Palliative Care nurse specialist.
  • Receive and process mail and telephone messages on own initiative as far as possible or in accordance with instructions.
  • Work flexibly to maintain an efficient service.
  • Offers support either face to face or via the telephone to patients and families who have been diagnosed as being in the last 12 months of life In either an inpatient or outpatients setting
  • Provide basic telephone advice and refer on or signpost to other sources of support
  • Demonstrate the ability to recognise and respond appropriately when faced with a sudden deterioration or an emergency situation, alerting the team or enabling rapid response as appropriate.

  • Manages and coordinates the Palliative Care Multidisciplinary team meetings.
Records and disseminates outcomes. Provides data collection to support Peer Review / QST measures

  • Responsible for maintaining the continuous accuracy of data as provided by team. Providing reports regarding team activity and dissemination in an understandable format.
  • Responsible for maintaining the accurate location, retrieval and recording of patient information on the systems used. Provides clinical records/information to support team referrals.
  • Responsible for ensuring all results from analysis are brought to the attention of the Palliative Care Team. Presenting results to appropriate clinical meetings.
  • Prioritise own workload as necessary. Identification of urgent referrals and highlighting to the appropriate Palliative Care nurse specialist.
