Job summary

The post holder will provide and facilitate a planned programme of therapeutic activities for the benefit of offenders under the care of the Neuro Developmental Team (NDT).

The post holder will contribute to the multi-professional team and to the process of assessment of care needs and the development, implementation, and evaluation of reasonable adjustments the individual programmes of care for all patients.

Main duties of the job

Under the supervision of the team lead, the post holder will:

Use technical, creative, and psychosocial skills to assist in the provision of a flexible, person-centred approach activities for those with neuro divergent needs requiring additional support.

Keeping and maintaining an electronic data base with respect to service user numbers, numbers of those screened, waiting list and on care pathways overseen by the team.

With colleagues, contribute to the development and quality improvement of the service.

Have an awareness of social inclusion, recovery and skills development and ensure activities used focus on these objectives.

Help the service user to gain access to resources.

Provide information on health promotion.

support service users with involvement/participation with their treatment.

Work independently and autonomously supported by the clinical team to provide a range of well-being educational and practical interventions, promote social inclusion.

Be part of the wider mental health team with a focus on the direct needs of identified clients to assist their recovery and to support them to engage with management plans agreed through case reviews by working across boundaries of care, prison departments, organisations and roles.

Job responsibilities

The post holder will:

Work independently and autonomously supported by the senior Learning Disability Nurse in developing person centred care, annual health checks and triages.

Be part of the team and focus on the direct needs of offenders as identified through triage and further assessment.

Raise safeguarding concerns if needed and highlight potential challenges in the care being given. Promote person centred care and empowerment.

Work alongside other healthcare and custodial staff.

Be able to give thorough hand overs and develop care plans and review risk assessments.

Contribute to multi-disciplinary care planning and evaluation from other professionals perspective.

Work in partnership with others to promote health and well-being, social inclusion and annual health check.

To conduct triage, risk assessments and welfare checks.

To liaise with LD nurse regarding best practice of care plans, communication passports, annual health checks and discharge summaries.

Promote person centred care and therapeutic engagement with service users, clinical and non-clinical staff.

Provide practical support using reasonable adjustments and a multi-disciplinary approach.

To actively participate in supervision and appraisals.

Be up to date with both clinical and mandatory training.

Reports to the Learning Disability Nurse.
