Our established Specialist Nursing Service is looking for a motivated and enthusiastic individual to work as a Community Parkinson's Disease Nurse.

The post will provide specialist support, assessment and treatment to adults with Parkinson's Disease across Southampton City in collaboration with the Specialist Parkinson's Disease Nurse and the broader Community Neuro and Specialist Services. The ideal candidate would have experience of working within a community setting with patients with a Parkinson's' Disease diagnosis and should have a genuine interest to specialise within this area. Formal training within this specialist area would be preferred. The role will work in both an outpatient and community setting and access to a car is essential for the role. Monday to Wednesday working would be preferred, with flexibility to work Wednesday to Friday initially to support the induction process but working days could be negotiable. Main duties of the job To assist the Community Parkinson's Disease Specialist Nurse in the provision of the specialist nursing service for all adult patients with Parkinson's Disease across Southampton City. To manage a caseload of clients requiring specialist Parkinson's Disease nursing, within the community. Undertake assessment and treatment of clients within both clinic and community setting, who have complex / and or chronic presentation requiring specialist skills and determine profession specific and multidisciplinary rehabilitation interventions. To provide advice, education and support to patients, their carers and healthcare professionals, and assist in coordinating treatment and care. To work across professional, departmental and Trust boundaries to support the delivery of a patient-centred seamless service. To work in close collaboration with the Community Neuro & Specialist Services, delivering and evaluating care pathways for clients with long term neurological conditions. About us We have been working with other local NHS Trusts to create a new organisation, bringing together all community, mental health and learning disability services from across Hampshire and Isle of Wight. Subject to approvals, the aim is for the new Trust to be formed by July 2024.Project Fusion is the name for the programme to create the new combined NHS Foundation Trust. Hampshire and Isle of Wight Healthcare NHS Foundation Trust will be bringing the services into a single organisation will result in more consistent care, more equitable access to services irrespective of postcode, and a more sustainable workforce and services. The new organisation will operate locally to ensure services can best meet the needs of different communities.The new Trust will be comprised of all the services currently provided by Solent NHS Trust and Southern Health NHS Foundation Trust, the community, mental health and learning disability services provided by Isle of Wight NHS Trust and child and adolescent mental health services delivered in parts of Hampshire by Sussex Partnership NHS Foundation Trust.Colleagues will transfer to the new organisation under Transfer of Undertakings (Protection of Employment) -- known as TUPE on 1st July 2024.   • Registered General Nurse (RN, RNLD) or equivalent other NMC
  • Degree or Diploma in Nursing
  • Portfolio evidence of post registration learning
Desirable
  • Evidence of post registration learning in Parkinson's Disease
Experience Essential
  • Experienced in managing patients with Parkinson's Disease
Desirable
  • Experience of working within the community setting
  • Experience of MDT working
  • Experience of working with people with learning disabilities and/or mental health conditions
  • Involvement in regional or national clinic bodies e.g. clinical interest groups, conferences etc
Additional Criteria Essential
  • Clinically competent at Band 6 level.
  • Excellent communication skills, written and verbal ability in English language, and report writing.
  • Verbally able to reason, articulate and manage challenging situations.
  • Ability to receive and provide complex information which may be sensitive, contentious, where persuasive negotiation may be required and there could be barriers to understanding.
  • Ability to work autonomously prioritise workload, instruct and direct others, work is managed rather than supervised works within clear code of practice
  • Understand the legal responsibilities of the profession.
  • Able to organise, prioritise and delegate.
  • Numerate.
  • Understand clinical governance and its implications for services. Including experience of quality issues and audit.
  • Ability to recognise when to seek advice.
  • Ability to keep accurate and legible patient notes.
  • Excellent IT skills.
  • Presentation skills.
  • Ability to work as part of a close-knit team.
  • Self motivated, self-starter, able to organise a large and varied work programme.
  • Able to be flexible and adaptable at work in order to meet competing priorities.
  • Ability to cope working in a stressful environment and with emotional or aggressive patients and carers.
  • Broad knowledge and experience of Parkinson's Disease.
  • Sound grounding in care of the patient with neurological conditions.
  • Knowledge of audit.
