Job summary

Bank pharmacist Band 8a is required to work weekends/bank holidays when required.

Provide an advanced clinical pharmacy service in accordance with professional standards and departmental policies. This will include:Take patients drug historiesReview medicines to ensure safe and effective use; including review of prescriptions for completeness, appropriateness and legality, problem identification and solving, questioning prescribers decisions based on prescription analysis where it may be challenged or opinion differsTo communicate highly complex recommendations on drug therapy to doctors, nurses, other relevant healthcare professionals and patients as appropriate where the recommendation made is based on analysis of highly complex information that maybe unavailable, incomplete or where there may be resistance to accept the recommendation.

Responsible for resolving medicines supply issues for designated area

Ensure safe and accurate transfer of prescribing information across care on therapeutic drug level monitoring for specific drugsInform, advise and educate patients about their medicines

The ability to speak Welsh is desirable for this post; Welsh and/or English speakers are equally welcome to apply.

Main duties of the job

Lead a safe and effective specialist clinical pharmacy service to allocated area

Undertake professional and clinical pharmacist's duties

Provides expert pharmaceutical advice and support within both primary and secondary care services as required.

Develop, encourage and promote evidence-based, cost effective use of medicines.

To be involved in the training of diploma, pre-registration and other pharmacists in area of specialty.

Assist in providing clinical training and supervision of pharmacy staff and other healthcare professionals.

To be involved in the development and delivery of practice research, service and quality improvement

Develop policies, protocols and guidelines within allocated area. Work in partnership with service users, other health and social care colleagues, to achieve the optimum health outcome for service users.

Person Specification

Qualifications / Knowledge

Essential

MPharm degree or equivalent Registered with the General Pharmaceutical Council. Post Graduate MSc/Diploma in Clinical Pharmacy or equivalent experience and/or qualifications

Desirable

Member of the Royal Pharmaceutical Society & registration with the Faculty Accredited or eligible to be accredited as a Higher Education Institute Clinical Pharmacy diploma/MSc tutor Training qualification Leadership and/or management qualification

Experience

Essential

Must be able to demonstrate suitable level of experience working in the managed sector and/or community pharmacy

Desirable

Experience in relevant clinical area Leadership and management experience
