Job summary

An exciting opportunity has arisen for a Young Adult Mental Health Practitioner. This post has been created to enhance existing pathways into adult mental health services for young people. The post holder and the pathway senior will work alongside the adult Mental Health and Wellness Teams providing locality based care within the London Borough of Redbridge.

The post holder will support new ways of working to shape transition care pathways in supporting young adults in preparing for adulthood. They will work with young people transitioning from children's to adult services, and with new referrals of young people into adult mental health. Based in adult services, they will liaise closely with CAMHS to ensure high quality, timely, accessible, evidence based support, improving service user outcomes.

The post holder will involve in assessment of newly referred client into the pathway including urgent assessment where possible.

The post holder will carry a caseload of clients for key working/care coordination.

This post is a pathway embedded within the MHWT and work across the three Redbridge MHWT.

Main duties of the job

This positionwould suit someone with a passion for supporting young people with their mental health journey, focusing on recovery and integration with other community services. The post-holder will be a qualified practitioner ( mental health nurse, social worker or occupational therapist), fully registered with the appropriate professional or regulatory body ( HCPC, NMC or Social Work England). They will be responsible for delivering support within borough based CAMHS and Adult Mental Health care pathways, receiving supervision and safeguarding supervision from a senior mental health professional in the service.

Starting with NELFT

NELFT place a great deal of importance on new starters being properly welcomed and inducted into the Trust. All new starters will join the Trust on the first Monday of each month and will undertake a comprehensive induction of up to two weeks which will include mandatory training, systems training and the allocation of equipment. As part of the process new starters will have the opportunity to also meet the executive team, senior managers and attend a number of drop-in sessions focusing on engagement, health and wellbeing and key processes. The induction will be held at our head office in Rainham, Essex.

Probationary Period

This post will be subject to a probationary period. Internal applicants are exempt from the probationary period (unless you are an internal applicant currently part way through a probationary period or currently a bank member of staff).

High-Cost Area Supplement

This post also attracts payment for High-Cost Area Supplement of 15% of the basic salary (minimum payment of £4,313 and a maximum payment of £5,436) pro rata for part time staff.

Key Responsibilities:

To routinely measure clinical / practice outcomes and deliver evidence-based interventions for 18- to 25-year-olds. To be responsible for supporting young adults who require adult mental health services, working alongside other services. To work in co-operation with members of the Multidisciplinary Team and other relevant health and social care professionals to assess, develop, implement and evaluate programmes of care to meet the needs of young adults requiring further support from adult services The post holder will also ensure that there is a clear transition plan developed for the young person To support and empower the navigation and introduction of adult health care systems To initiate and complete risk assessments following Trust policy and guidance to ensure that complete and up to date risk assessments are considered when planning programmes of care. To contribute to the effective functioning of the service through attendance at relevant meetings, taking responsibilities within the service as appropriate and negotiated with the service manager. To ensure effective working approaches are provided to service users and that these are informed by digital resources where appropriate. To alert senior management and take decisive action to mitigate any risks associated with individual service users or servicewide delivery of care. To develop individual case formulations in conjunction with the MDT, in order to plan interventions for service users and collaboratively engage them in their care and goals from the outset of leaving CAMHS/ CYP services and entering into Adults Services. To work in an assertive outreach way where required to minimise the risk of YP/YA falling between services. To carry a caseload of clients for key working.

Experience of working with highly complex young people and adults requiring skilled and complex interventions. Experience of working with children and adolescents and adults with co-morbid difficulties and special needs. ( combination of organic and emotional deficits, such as autistic spectrum disorders and trauma). Experience of working therapeutically with parents/carers/families of children and young people and adults with complex mental health problems Experience of carrying out generic assessment with other colleagues in the multi-disciplinary teams Experience of assessment carried out autonomously in order to determine the most appropriate treatment plan from a range of options for the child/young person and adult.
