Changing Futures is one of 15 national Programme's bringing together public and community sector partners to research ways that local systems and services could be delivered, designed or commissioned to meet the needs for adults experiencing multiple disadvantage. Multiple disadvantage is defined by the national Changing Futures Programme as 'people who experience three or more of: homelessness, current or historical offending, substance misuse, domestic violence and mental ill health'. The Changing Futures Programme Sussex aims to create an environment where individuals experiencing multiple complex needs (MCN), who repeatedly fall through the net of service provision, receive flexible, trauma informed, person-centred support. The Programme aims to enable positive changes in services; to make them better connected, and easier to access. Changing Futures Sussex works in partnership and is embedded across the three Local Authority areas: East Sussex, West Sussex, and Brighton & Hove.
About The Job
Salary: NJC Grade 8 £30,296 to £33,024 per annum
Working Pattern: Full-time, Mon - Fri, 9am-5pm (37 hours per week)
Contract Length: Fixed Term until 31st March 2025*
Location: Office base Bridge House, Worthing and travel around the county as required
You will be part of a multi-disciplinary team in West Sussex delivering trauma-informed, person-centred support for clients. You will adopt a curious, creative and collaborative approach to service provision, fostering relationships with professionals to ensure that clients receive wrap-around support. The team will inform Changing Futures Sussex work to identify blocks and barriers for clients within the system, recognising best practice and working flexibly to overcome challenges for clients you are supporting.
As a Mental Health Prevention & Wellbeing Officer:
- You will have a distinctive contribution to make to the current challenges faced in mental health services.
- You will need to fully understand and implement a strength-based approach that will ensure that all clients are supported in this manner. This approach would be applied in all elements of social care including assessment, planning, implementing, and review of clients care.
- You will need to demonstrate an in-depth knowledge of The Mental Capacity Act and how to apply it to Social Care, while ensuring West Sussex County Council safeguarding procedures are followed.
- You will be working with Social Workers closely across services, both internally within Adult Social Care, and externally with our partner organisations such as Sussex Partnership Foundation Trust and other third sector agencies. To do this you will need the ability to maintain and build on relationships with several agencies to ensure effective support is provided to customers using mental health services.
- You will be working with clients who need intensive support that will enable them to live meaningful and independent lives. You will be supporting clients in the community and those being discharged from hospitals and will require the ability to work under pressure.
- You will work with clients with specific mental health difficulties, in conjunction with either: experiences of domestic violence, substance misuse, contact with the criminal justice system or homelessness.

- Ability to develop and deliver innovative solutions for individuals, to ensure they can engage with community assets and minimise the need for long-term formal support.
- A passion for enabling people to live independent and meaningful lives that meet their personal aspirations.
- Excellent communication and interpersonal skills to interact effectively with a range of stakeholders to provide clear advice and guidance.
- Ability to recognise the limits of own accountability and responsibility, and to know when to seek appropriate support, advice and supervision.
- Ability to develop relationships within a multi-agency working environment that has contributed to improvements to individuals and or services.
Qualifications and/or experience:
- GSCE English and Maths, or equivalent qualification/experience demonstrating that level of literacy and numeracy.
- Experience of working within adult social care, or a care provider or health provider, with experience of working with individuals who have complex mental disorder.
- Up to date knowledge and experience of safeguarding, assessment and review processes, policies, and legislation.
- Working knowledge of financial eligibility for statutory services and benefits.
- Knowledge of a variety of associated legislation, for example Care Act, Mental Capacity, Mental Health Act.
- Experience of forming effective working relationships within a multi-agency setting.
