Purpose of the post

Ensure that the people of Gwynedd are at the heart of everything we do.

To enable disabled children and young people and their families to develop skills and to promote their independence by working with them in a practical way.

Responsibility for functions

No direct responsibility but the successful candidate is expected to be responsible for enabling staff and for the team’s equipment.

Main duties

General


Responsibility for one to one work and in groups :-

owith the families of disabled children

owith disabled children and young people

Working in an Integrated maner with a variety of professions within and outside Derwen to provide a purposeful and quality service for families.

To ensure a continium of relevant programmes and outcome based interventions to meet the needs of the service user.

To carry out detailed assessments of need and to prepare a care plan to respond to the need, in conjunction with the service user, his / her family and other professionals.

To establish and maintain systems to monitor the provision through measuring the effect of the programmes and to ensure that the performance management systems are effective.

To create information and data reports in a timely and tidy manner and to ensure that all of the needs of all of the outside agencies are met in relationship to progress and monitoring work programmes.

To contribute towards developing and implementing the Participation Strategy to ensure that parents, children and young people take part in the work of planning, developing and providing their own services.

To attend and to actively contribute towards a range of relevant meetings.

To prepare relevant Information.

To provide careful management of any resources given to the team.

To keep electronic records of interventions.

To communicate effectively verbally and in writing through the medium of Welsh and English.

Corporate responsibilities

To attend Derwen meetings.

To attend and to make positive use of professional and operational supervision sessions.

To fulfil any other corresponding duties and that which can reasonably be expected to be fulfilled in accordance with salary level and responsibility of the post.

Responsibility for self-development.

Ensure compliance with Health and Safety rules in the workplace in accordance with the responsibilities noted in the Health and Safety at Work Act 1974 and the Council’s Health and Safety Policy.

Operate within the Council’s policies in relation to equal opportunities and equality.

Responsible for managing information in accordance with the Council’s information management guidelines. Ensure that personal information is treated in accordance with Data Protection legislation.

Commitment to reducing the Council’s carbon emissions in accordance with the Carbon Management Plan, and to encourage others to act positively towards reducing the Council’s Carbon Footprint.

Undertake any other reasonable duty which corresponds to the salary level and responsibility level of the job.

Responsibility to report any worry or suspicion that a child or vulnerable adult is being abused
