Able to follow a conversation or discussion through the medium of Welsh and English on a professional level and discuss general day to day topics in the field in order to present information and express opinions.Able to give a pre-prepared presentation and respond to any comments and questions on it in Welsh or English.

**Reading and Understanding - Higher Level**:
Able to understand standard written Welsh and English; both formal and informal.Able to gather information from various sources such as letters, reports, articles through the medium of Welsh and English in order to fulfil the post.

**Purpose of the post**:
“To enable me to live my life as I want to live it”.
- Assist vulnerable individuals to live their lives in the way they wish to live them.
- Undertake complex manual handling assessments and manual handling reviews, facilitate timely hospital discharge, promote right sized care, and provide the relevant training to individuals, their families and care staff.
- Ensure that the people of Gwynedd are at the centre of everything we do.
- Assess the needs of the individual and their carers in accordance with the requirements of the Social Services and Well-being Act 2014 and other relevant legislation and guidelines.
- Provide an Information, Advice and Assistance service to citizens to enable them to meet their personal outcomes.
- Assist people to identify solutions, prevent dependency and promote independence
- Provide information about the support resources that are available in the community, from the third sector, the Council and the Health Board, to enable citizens to live as independently for as long as possible and to maximise their quality of life.
- Ensure that individuals' needs are met in accordance with the Council's safeguarding arrangements and statutory responsibilities.
- Ensure that the people of Gwynedd are at the heart of everything we do.
