This post is a new permanent 10 PA consultant position based in the Emergency Department of the Royal Bolton Hospital. The successful applicant(s) will join 15 other consultants to provide comprehensive clinical support to the medical and nursing staff.

The consultant rota is self-rostered and annualised, to allow for significant flexibility and better work-life balance, but that there is also an option to have some fixed shifts if that is preferred.

The Royal Bolton Hospital Emergency Department continually seeks to develop and improve the service of emergency care to the local population. The Department has a strong educational background, with an established programme of undergraduate and postgraduate training as well as Consultant representation at local, regional and national level. All Consultants have a variety of special interests that include Paediatric Emergency Medicine and Event Medicine. The role will also require successful candidates to play a lead role in the improvement programme of the department including Rapid Assessment and Triage and supporting the development of the Urgent Treatment Centre

We therefore seek an enthusiastic and ambitious colleague who will share and promote these objectives and develop or expand their own special interests to compliment the Department.

Main duties of the job

Clinical Duties - Clinical duties will be those normally expected of an Emergency Medicine consultant in a large NHS Acute Trust in the United Kingdom. The objective of this appointment is to provide enhanced clinical supervision, to support the 24-hour intermediate grade cover, team of junior doctors and nurse practitioners. This will require a consultant to be readily available on the 'shop floor' during the weekdays and for periods as arranged at the weekends.

General follow-up clinics and ward rounds take place each weekday morning every consultant will be expected to participate in these duties.

Full discussion with the successful applicant will take place prior to the implementation of a job plan (in accordance with Terms and Conditions of Service).

About us

Bolton NHS Foundation Trust is a major provider of hospital and community health services in the North West Sector of Greater Manchester, delivering services from the Royal Bolton Hospital and also providing a wide range of community services from locations across Bolton. The Trust received a 'Good' overall rating from the CQC in July 2016.

Royal Bolton Hospital comprises approximately 800 in-patient beds including 22-day care and 9 endoscopy beds. There is a 7-bedded intensive care unit a 6-bedded HDU, a 7-bedded CCU and a 100-bedded maternity unit.

The Emergency Department has recently undergone modernisation to provide a new 6 bedded resuscitation area, ambulance handover area, minors department, new staff facilities and two seminar rooms.

There is a separate Paediatric Emergency Department which opened in late 2007 (see below).

24-hour pathology and diagnostic services including 24 hour 7 day a week CT scanning as well as MRI scanning on an in hours 7 day a week service.

The Emergency Department has its own dedicated USS machines allowing USS and echo dependent upon the operator's skill level. The USS service is supported by a dedicated team of ultrasonographers who provide detailed scanning during the week as well as educating ED staff.

Job responsibilities

The unit serves the population of Bolton and surrounding districts with an estimated population of 330,000. New patient attendance for 2021/22 averages at 380 per day and shows a typical caseload from a mainly urban population, with several areas of deprivation and an ethnic population of over 10%. The ambulance attendance rate is over 25% of new patient workload and reflects a high incidence of acute medical problems. The department admits approximately 20% of new patients and 25% of the case mix are children under the age of 16.

The department operates 24-hour nurse assessment and has a well-established 24-hour nurse practitioner service.

Facilities

Reception - This area is staffed throughout 24 hours, with a dedicated receptionist in Paediatrics at least 12 hours per day.

Waiting area The area includes 24/7 security, refreshments, TV, toilet facilities, security cameras and police room. A dedicated RN is allocated to the waiting room to help manage ambulatory patients.

There are two separate facilities for distressed relatives of patients in the resuscitation area. Patients attending the A& E clinic have a separate waiting area.

Clinical areas - All clinical areas are equipped to a high standard and consist of:

The Children's Emergency unit, provides an audio visually separate purpose-built area, a safe, efficient and convenient service for the emergency care of sick and injured children and young people up to the age of 16. The children's clinical areas are also equipped to a high standard and consist of:

Other accommodation - There is office space for the consultants and offices for the matron, nurse consultant, senior nurses, operational business manager, secretariat and trainees. There is a staff rest room and a separate teaching / seminar room. A small number of Emergency Department observation beds are located within the main hospital.

Medical Staffing

GPs

Nursing

Our nursing teams are led by a Nurse Consultant and Matron for the department. We have a number of senior nurses that will work alongside the consultant in charge to ensure the department is safe at all times and we maintain flow/performance.

Our department benefits from the experience of Cardiac Nurses and Advanced Nurse Practitioners

Our minor injuries area is staffed by Emergency Nurse Practitioners and Nurse Associates. There is a cohort of senior nurses who can perform Emergency Nurse Practitioner duties to pre-determined protocols which provides a constant 24-hour minor injury service.

Many of the nurses have completed Advanced Life Support Courses (Trauma and Paediatric) and there are several development groups looking at aspects of paediatric care, elderly care, quality and audit and bereavement support.

We are currently expanding our RSCN numbers to meet our clinical needs. The Department has access to alcohol and drugs liaison nurses, a 24-hour mental health crisis intervention team, a Home First OT/Physio service, plus in-reach from Acute Medicine, Care of The Elderly, and Acute Oncology physicians and ANPs.
