Futures+ is an Adult Specialist Team. Following a recent redesign and expansion of the team we have an exciting opportunity for the right person to join our Community Inclusion. We are recruiting two full time Enabling and Transitioning Officers. We are a friendly and dedicated team with diverse experiences, knowledge, and skills. Our team won Social Worker of the Year 2023.

We are looking for practitioners who are committed to equality with good communication skills to join our team in our journey for continual transformation. Your main responsibilities are to work with service users and their carers to assess and review their needs in accordance with the Care Act 2014. You need to be creative in your communications and be kind, respectful and resilient. We strive to support our service users to achieve best outcomes for them. We welcome applicants from different sectors. You will be supported to learn how to complete the assessments and reviews. This is a very exciting opportunity for practitioners who want to expand and develop their skills; and to take the next step in their professional development within a well-established local authority.

You are expected to carry a caseload and work as an autonomous professional, applying your knowledge and skills. This will include working with other professionals. You will be a part this brilliant team to work with service users and their carers to support and enable them to stay safe, be healthy, have aspirations in education or employment, and to achieve independency.

You will be supported to learn and apply relevant legislation such as the Care Act 2014 and the Mental Capacity Act. You will have training opportunity to upskill and learn about different approaches to Adult Social Care such as the Strength Based and Person-Centred Approaches. Collaborative working is an important part of our works; therefore, you are be supported to work with multi-agencies.

As the role involves interacting and collecting information from service users and their carers, you must have excellent communication skills, the resilience to work under pressure and be flexible in your approach.

In return, the successful candidate can expect to work as part of a strong and caring team, receive regular professional supervision and have access to quality training, develop your interests, expand your knowledge, and become an expert at what you do.
