Job overview

We are looking to appoint an experienced, enthusiastic and motivated RNLD who is committed to supporting the needs of people with learning disabilities as an Acute Liaison Nurse for people with learning disabilities at Royal Shrewsbury Hospital.

The role requires someone who is able to demonstrate excellent inter-personal communication skills and a determined commitment to person-centered, multi-disciplinary working.
This post offers the opportunity to work in a highly focused and supportive way with people who have complex physical health needs when admitted or discharged from hospital. 

The post holder will be responsible for providing guidance to acute hospital staff on meeting the needs of people with learning disabilities in The Royal Shrewsbury Hospital  and on occasions, Princess Royal, advising on reasonable adjustments to receive appropriate care and treatment.

Experience of working with people with learning disabilities in the community is desirable. You will also preferably have experience of working with people who have complex needs, for example those associated with mental and physical ill health, autism and behaviours that challenge. 

The role involves close collaborative working with the acute trust including awareness raising, identifying and meeting training needs and the provision of specialist nursing advice. You can expect specialist clinical training, development opportunities, regular supervision and professional leadership and a warm welcome.

 

Main duties of the job

• To be responsible for community or hospital admissions, including liaison with other agencies, service users and carers.

• To evaluate the outcome of nursing interventions to ensure that care plan objectives have been met and that health gains have been achieved.

• To co-ordinate care with the MDT and hospital staff at points of attending appointments, admission and discharge.

• Any interventions must be within the post holder’s scope of clinical practice ensuring that training is maintained and skills are in line with current guidance.

• To provide advice, practical support and guidance with regards to people who may lack capacity to make decisions about their health.

• To develop risk assessments if needed and appropriate risk management plans.

• To undertake or facilitate assessments of patients before admission.

• To promote awareness of health issues and the development of health action plans with service users and their carers. 

• To support the acute hospital services in the assessment, planning, implementation and evaluation of care plans for service users with highly complex needs.
