Join our team as a Mental Health Homeless Prevention Worker, playing a pivotal role in supporting individuals within the Leeds & York Partnership Foundation Trust (LYPFT) – CMHT community. You’ll work collaboratively within an inclusive recovery-focused approach, you will work closely with Accommodation Gateway, Care Coordinators, and other housing support services to facilitate housing transfers, resettlement and secure housing options.

If you have

· Strong communication and interpersonal skills.

· An understanding of the Care Programme Approach (CPA).

· The ability to work professionally with a client focus.

· A full UK driver’s license and access to a vehicle for work.
