Connect2socialwork are urgently looking for an Adults Service Manager to join their great Adults Hospital Discharge Social Work Team's. Paying Up to 450 a day, this role is on a locum basis for an initial 12 month rolling contract.

Role Responsibilities:

  • The Service Manager will be responsible for leading a city wide hospital discharge service working closely with system partners to reduce ength of stay in acute settings and promote the home first ethos.
  • The post holder will support in the development of the Transfer of Care Hub and Pathways 1, 2 and 3.
  • You will work collaboratively with service users and carers groups and staff at all levels within the service and partner organisations to develop effective reablement and strength-based services that focus on improving the outcomes of people and achieve best value.

Requirements as a Social Worker include:

  • SWE Registration
  • Eligible to work in the UK
  • Drive and have access to a car
  • Post-qualified experience
  • Degree level or equivalent in Social Work
