Job Description: Digital Navigator – Adult Social Care

Location – Birmingham and Wolverhampton region including hybrid working.

Context

West Midlands 5G Limited (WM5G) is the UK’s largest 5G innovation company. Since its inception in 2018, WM5G has achieved considerable success in helping the West Midlands become the best-connected region in the UK for 5G and in testing and proving transformational benefits from breakthrough 5G services in healthcare, transport and manufacturing.

Following WM5G’s initial success, the organisation continues focussing on accelerating the deployment of advanced connectivity, delivering further innovation and support organisations to adopt the benefits from connectivity, internet of things and data & AI solutions. The mission is to make it radically simpler for public and private sector organisations to get the benefits from these new technologies – building on the real-world expertise and experience from WM5G.

This role is to support two specific programmes called ‘5G Innovation Regions’ and ‘Smart city Region’, where we are working with Local Authorities to deliver improvements in Adult Social Care by introducing wireless connectivity solutions through a scale up programme.

Purpose

The Digital Navigator role will work directly for WM5G but will be integrated within the Local Authority (Birmingham or Wolverhampton) Adult Social Care Services, supporting the Social Care professionals to understand and best utilise the TEC available. The role will support the frontline delivery of the council’s Digital Transformation agenda. The Digital Navigator role will support Adult Social Work teams, helping identify Social Care recipients who could benefit from technology enabled care interventions. This will be done utilising a person-centred approach to assist Social Workers in developing personalised care plans.

The aim of this support is to find ways to replace traditional forms of social care support with new digital innovations that can help reduce unnecessary presentation to unscheduled healthcare, increase independence of service users and allow service users to better realise the positive outcomes detailed within their care plans.

You should have a working understanding of the processes and functions of adult social care and a knowledge of digital technologies that can support service users to live lives that maximise their independence and quality of life. You should also have demonstrable analytical and IT skills and be able to produce reports, data and information for a variety of purposes and audiences.

Accountabilities

To support the Social Care professionals to assess the referrals and to understand what TEC solutions provided by the TEC Supplier are best suited to the care challenge. To help establish the appropriate and best TEC solutions for each health and care need, based on those available from the Supplier. To provide insight and data to the LA’s and WM5G on which device solutions work effectively and produce efficiency savings, and benefits, and those that don’t. Support teams to consider technology at all stages of care pathways and ensure a technology-focused approach is embedded in social care practice. Attend team meetings and host demonstration sessions. Coach teams during the completion of initial contacts, care needs assessments, risk assessments, and care plans to consider technological alternatives to supporting care. Promote learning and upskilling around the use of technology within teams so that they become self-sustaining. Be aware of risk management and escalation routes and be confident in managing and escalating risk as needed to promote the safety and security of people using the service as well as Care Act compliance. Work with Social Worker teams to ensure teams have the most accurate, timely information at their fingertips to provide outstanding care. Develop a core offer for social workers most in need of education around the benefits of the technologies and support in are removing blockers and implementing them. To undertake co-production and sharing of best practice with and between providers and social workers in order to develop and shape the process of digital navigation, utilising existing and newly established forums and networks. Effective self-starter able to work on own initiative to tight deadlines where there are often conflicting requirements and requests for support from other parts of the team.

Experience

Past experience of working as a Social Care or Health Care professional and the utilisation of technology to support health and social care. Experience researching and trialling assistive and communicative technologies. Knowledge of how care planning should take place for individuals in receipt of support services. Experience in improving independence within or around a care setting. Experience of working in a change management environment.

Skills

Highly proficient within the social care setting. Have an interest and knowledge of technology that can be used to improve health and social care. Strong influence and persuasion skills, re-enforced by persistence and resilience. Excellent verbal, written and presentation skills, preparing and delivering effective communications.

· Exhibit a high degree of pro-activity and creative thinking when faced with challenges requiring resolution.

Qualifications

· NVQ Level 4 in Health and Social Care or equivalent qualification

· Relevant professional qualification or significant relevant professional experience.
