About The Role

This is an exciting role working closely with mental health services to support our most vulnerable service users.

You would be working alongside;

  • Carers,
  • Community social care teams,
  • Community Mental Health Teams,
  • Health colleagues,
  • Care providers,
  • The third sector

You will be tasked with delivering strengths-based support; to enable safe and timely discharges and working with customers to prevent hospital admission.

You will be supported to undertake strength based and person-centred assessments in line with the Care Act and Mental Capacity Act and you will support colleagues within Adult Social Care such as the safeguarding team to ensure the wellbeing of everyone you work with.

About You

It goes without saying that it is essential that you can show sound knowledge of relevant legislation underpinning statutory Adult Care Practice, Social Work theory and be able to demonstrate values that promote wellbeing, challenge oppression and discriminations and empower people to live well.

The post holder must hold a professional Social Work qualification e.g. DIPSW, CQSW, CETSW, CSS or BA Social Work and ensure that registration with Social Work England is maintained and renewed appropriately.
