We are seeking to recruit an experienced general practitioner who will is capable of leading a service, developing that service and enabling all those within the service team to provide the best care possible to their patients in line with the Trust strategy.

Main duties of the job

Work autonomously and be accountable for their own professional actions Maintain and comply with the requirements of registration with the GMC and maintain registration on a performers list in England Undertake clinical practice at an advanced level using expert knowledge and clinical skills to deliver holistic care to people accessing primary care services Engage in all usual administration activity associated with day-to-day general practice Make onward referrals where appropriate Undertake home visits as appropriate Undertake telephone triage, video and telephone consultations as needed Keep up to date with clinical developments, local and national guidance As a prescriber engage in all usual activity to ensure safe prescribing Maintain high standards of record keeping Engage in clinical audit, quality assurance processes and medicines management activity as necessary Undertake all duties and responsibilities associated with a GP working within primary care, including a full range of medical services as defined in the core PMS/GMS contract and additional and enhanced services where appropriate Lead on coordinating practice work to achieve the best clinical performance possible in areas such as QOF performance, national vaccination and screening programs. Develop action plans to address any issues regarding clinical performance

Clinical Responsibilities

The clinical lead GP will be indemnified by the Trust for all clinical work carried out in this role. Strategic Management and Planning

Contribute to the management of transformation within the practice in line with the changing needs of the practice population Help the practice to align with the ELFT Trust values and lead on developing practice processes to allow this to happen Contribute to developing the role of the practice within the local PCN and also the wider STP including developing professional links with clinical colleagues across the PCN and STP Lead the practice participation in PCN work and contribute to work to deliver contracts such as the PCN DES Lead on the development of priorities for clinical work within the practice Contribute to work within ELFT to integrate primary care, mental health and community services

Staff Management

Providing local support to the practice clinical team with respect to training, mentoring and strategic role development Working as a liaison between the practice team and the primary care directorate management team Participate in the recruitment of new practice clinical staff Act as a line manager for the salaried GPs

Organisational Responsibilities

Helping the practice to adopt and maintain relevant Trust wide processes such as contributing to and taking part in directorate management team (DMT) meetings, quality assurance group (QAG) meetings, CQC readiness work, health and safety meetings, equality and diversity meetings, maintaining a risk register, developing and nurturing a quality improvement culture and being a leader for change Linking clinical work in the practice with the wider work in ELFT

Patient Services and People Participation

Contribute to work that ensures care is delivered by the surgery in line with NHS contractual obligations, working with local and national guidance Contribute to people participation strategies to align with the trusts vision for service user and carer involvement Contribute to a working together group with service users Help to foster a culture that prioritises patient and public involvement in all aspects work at the surgery

Information Management and Technology

Contribute to IT development with an emphasis on delivery of clinical services Keep up to date with and develop skills in primary care IT

Communication

Develop ways for the practice team to communicate effectively with one another Foster an open dialogue across the practice team that allows everyone to feel their voice is heard Strive to continuously improve the two-way communication with service users and carers and actively encourage people participation regarding clinical services Encourage members of your team to create their own networks across the rest of the Trust Develop strong positive communication links with the local population including voluntary organisations

Practice Leadership

Provide visible clinical leadership, in conjunction with the directorate management team and work to develop a stable, happy, functional environment that allows the practice to move forwards Lead and develop the GP team. Work with the Primary Care Lead Nurse to develop the nursing team Embody the trust values of we care, we respect, we are inclusive Actively encourage all members of the practice team to share leadership roles so that they feel empowered and engaged Maintain the best features of the practice whilst at the same time ensure it reflects the values of East London Foundation Trust Represent the practice externally and be outward facing e.g. working collaboratively within the local primary care network and STP, representing the practice and the Trust at CCG events, meetings with wider stakeholders etc Act as a link with the other ELFT primary care services and foster close working relationships across these services

Governance and Quality Assurance

Be the governance lead / Caldicott officer within the practice including assisting the Trusts Caldicott Guardian e.g reviewing consent overrides in clinical systems and reviewing and signing off SIs. Note the Lead is not required to be the CQC Registered Manager Develop clinical governance strategies, systems and processes and ensure the active participation by all medical staff Chair a monthly practice based clinical governance meeting and lead the implementation of improvement plans to address any quality assurance issues Attend the monthly directorate Quality Assurance Group and act as a link between practice and directorate level quality assurance processes Develop a good working knowledge of the Trusts quality assurance workstreams of service user led accreditation, CQC readiness, Trust wide audits, directorate audits and Patient Reported Experience Measures (PREMs). Lead this work with the practice team Lead work to respond to national patient feedback surveys and develop action plans in response to the survey results Help to develop and maintain a practice-based risk register with an associated action plan to address any risks Act as the lead clinician in the practice for the dissemination of alerts and planning responses to alerts as required Promote an organisational culture committed to learning from complaints, incidents, audit, research and development Engage with the trusts Datix program

Practice Management

Lead on clinical issues e.g.

a) Working with the HR team to resolve issues relating to the poor performance of medical staff and the implementation of disciplinary procedures

b) Ensuring appraisal, job planning and continuing professional development (CPD) takes place for doctors, nurses and allied health professionals

c) Monitor quality and appropriateness of clinical care

d) Peer support and review

e) Clinical protocol development

f) Correct clinical policies and procedures are in place

g) Clinical staff appraisal

h) Clinical audit and research

i) Recruitment, induction & appraisal of suitably qualified and experienced staff

j) Investigation of incidents and complaints

k) Clinical record keeping and documentation

l) Management of conflicts between clinicians

m) Management of clinician impairment or behavioural issues

Participate, in conjunction with the directorate management team, in planning and monitoring the practice budget. Develop plans to address any practice overspend in partnership with the practice manager Work with the practice management team to lead the practice team including ensuring effective communications and practice meetings Work with the directorate management team to ensure the practice offers high quality care and meets its agreed budget including revenue opportunities

Personal Development

Work with the Medical Director to design a developmental programme for the Clinical Lead role including mentoring Participate in annual appraisal and regular review meetings with the Medical Director Assess personal educational and training needs and explore all opportunities for maintaining and updating own professional knowledge and skills Take responsibility for personal development with support from the directorate senior management team Engage in the annual appraisal process and maintain a personal record of CPD activity Adopt an approach of lifelong learning, identify personal development needs and seek out appropriate training opportunities Engage in reflective practice and take opportunities to learn from both successes and failures. Develop opportunities to reflect as part of a team, actively encourage and lead this work across the practice clinical team Seek support as needed with any problems encountered Actively seek out opportunities to develop skills in quality improvement in line with the trusts QI work

Quality Improvement

Lead and coordinate the quality improvement work within the practice Ensure active participation of the practice team in quality improvement activity Link with quality improvement work across the Trust Help to develop and maintain the primary care directorate quality improvement forum
