About The Job

Salary: starting at Grade 6 £25,119 to £26,421 per annum, with the opportunity to progress to Grade 7 (£27,334 to £29,777 per annum) after 12 months and formal assessment by team manager.
Working Pattern: Full-time (37 hours per week) Working Monday and Friday (09:00 to 17:30)

Make your next career move one that cares for others, and makes a real difference to people's lives . We're looking for a Care Coordinator to deliver and provide high quality, fast paced support. You will work alongside our social work and health care colleagues responsible for delivering a statutory social work service and NHS service.

As a Coordinator, you will:

  • Make initial contact with customers, gathering information relevant to their health and social care needs, signposting where relevant to other agencies.
  • Ensure a prompt response to crisis referrals and coordinate interventions ensuring that referrals are made promptly to other services to prevent unnecessary hospital admissions.
  • Coordinate incoming referrals, triage, or signpost away referrals and present triaged referrals to the weekly Multidisciplinary Teams (MDT's).
  • Be IT literate and confident working across different IT platforms/systems

The nearest train station is Hassocks and the bus service in the local area doesn't start until 10am so the office location can be challenging to reach via public transport.

Experience and Skills

Please ensure you cover the key skills and qualifications/experience points listed below as they are the criteria your application will be short listed against.

Key Skills:

  • Ability to analyse and interpret complex information and prioritise needs in order to make recommendations/decisions which will have a positive outcome.
  • Ability to work in a multi-agency environment in order to deliver a service to others.
  • Ability to communicate, both verbally and in writing, complicated or sensitive information to a range of audiences to achieve positive outcomes. In particular, to be able to communicate clearly with customers reflecting the service objectives and standards.
  • Ability to prioritise and manage a workload with competing deadlines and in line with performance indicators and targets.
  • Ability to recognise the limits of own accountability and responsibility, and to seek appropriate support, advice and supervision.

Qualifications and/or experience:

  • GCSE English and Maths or equivalent qualification or experience operating at this level of literacy and numeracy.
  • NVQ Level 3 in Care - Desirable
  • An awareness of the policies, practices and procedures in relation to Social Work.
  • An understanding of the role as part of the wider community and voluntary sector.
  • Experience of providing support / working with people in a Social Care or Health environment.
  • Experience of forming effective working relationships within a multi-agency setting.
  • Experience of providing advice and guidance to others.
  • Experience of applying relevant legislation, policies and procedures to individual pieces of work.
  • Experience of recording sensitive and confidential information on IT systems.
