Thames Reach runs projects and services that help to end homelessness in London. We work directly with homeless people on the streets and in hostels. We work to prevent vulnerable people from becoming homeless. We help formerly homeless people get their lives back on track. We help people find work and access training and volunteering opportunities.
Last year we helped more then 12,000 people.
We are looking for a Support Worker to join the City Mobile Intervention Support team.
The Thames Reach Outreach teams operate across London and help those who are rough sleeping to find routes away from the street. Individuals are assessed to identify their support needs and support plans are created, including work to obtain identification, access health services and refer to other specialist agencies, including those offering immigration support.
Assess the support needs of new clients in a person-centred manner, using a standardised process.
Manage a caseload of clients, identifying the most appropriate route towards sustainable and permanent accommodation, and devise plans to achieve this.
Work alongside various partner agencies to ensure that clients are accessing appropriate support to access and sustain accommodation.
Work with clients who identify as women and develop specialist knowledge in women’s homelessness.
To succeed as the Support Worker you will have:

Have commitment to supporting people to achieve the best outcomes as they work towards independence.
Have the capability to carry out administrative tasks alongside practical support work, and manage and mitigate risk.
Have IT skills with the ability use Outlook, online recording systems, Excel, and Word, using a tablet and a smart phone.
