An exciting opportunity has arisen for a clinical nurse speciality with experience in multi-pathology management to join the POPS team at Guy's and St Thomas' NHS Foundation Trust (GSTT).
The proactive care of older people (POPS) team is an award-winning service and the first of its kind in the UK. It started in 2003 and has rapidly become an essential surgical support service. It is recognised locally and nationally as being of high quality, innovative and clinically effective. POPS looks after patients aged 65 years and above and aim to improve their medical health before and after surgery by assessing them before surgery, following their care while in hospital and supporting consultants and ward staff.
The post holder will have contact with the patient at all stages of the patient journey whilst also gaining experience in supporting and managing of the nursing and medical team. The post holder will be given the opportunity to support local and national audit on a number of ongoing projects whilst also undertaking their own audit of practice.
Outpatient setting, attending clinic weekly to
conduct comprehensive geriatric assessments to inform perioperative risks
Present the patients assessed in clinic, contributing to management plans

Inpatient surgical ward rounds
Review known POPS patients and case find at risk patients
Promptly identify and manage medical and multidisciplinary complications
Attend and develop surgical ward MDT meetings to
Improve quality of care for older people and facilitate discharge planning
Review & triage referrals to POPS, liaising with admissions and surgical colleagues
ensure high quality communication with the patient, relatives and carers
support patients and carers through their surgical journey
attend and contribute to LOS/Cluster meetings in Surgery
We are a multidisciplinary team who are friendly and dynamic working in a busy NHS Trust.
Using comprehensive geriatric assessment we assess older adults with complex medical issues and or frailty who may require surgery. Through evidence based practice we are able to identify potential medical complications, formulate management plans and support with shared decision making.

BSC/1st degree (health related) or equivalent qualification/training and experience
To have or working towards non medical prescribing qualification

RN1 or RN child registered nurse on the NMC register. Extensive experience of working autonomously at an advanced level within the specialist area delivering effective patient focused care.
Previous experience of student supervision and clinical education at undergraduate level
Ability to conduct history taking, physical assessment skills and interpret data to direct patient care and formulate plans
Proven listening and counselling skills with ability to manage complex situations with the ability to deliver sensitive difficult outcomes
Undertaken clinical & nursing research
Experience in the outpatient setting, conducting clinics/letter writing

Diplomatic, flexible and positive approach to work
