Currently working in care with experience of reviewing and assessment? If the answer is yes…We are seeking an exceptional individual to join our expanding “Trusted Assessment” team.
These roles don’t come up often, and when they do, they go fast
The appointed role holder will be responsible for the assessment and review of care and support services delivered to Service Users within our day time and night time services, and within extra care sites, joining a team of 3 x Trusted Assessors already in post.
This is a great opportunity for individuals looking to step up or use their existing skills and experience as an assessor to join us at a time when we our expanding our services.
Responsibilities:
* Working alongside the Trusted Assessors and Care Coordinators to review and respond to changes to service users care needs, providing a problem solving approach which is responsive and flexible.
* Using electronic care planning systems, review and amend risk assessments of new and existing service users ensuring delivery of ‘Person Centred Care’.
* Agree appropriate risk control measures to reduce identified risks during all reviews and assessments.
* Provide professional advice on all aspects of care, support and wellbeing.
Requirements:
* Hold a minimum of 6 months experience in a care setting either as a care coordinator, care assessor or care supervisor / manager with experience of assessment and review.
* The holder of a Level 2 QCF in Health and Social Care [minimum] or have achieved or be working towards a Level 3 QCF or above.
* Be proficient in the use of I.T systems.
* Holder of a UK Driving Licence with access to a vehicle.
* Strong communication and interpersonal skills
* Ability to work collaboratively in a multidisciplinary team.
Terms:
* Contract: Permanent - Full Time
* Hours: 40 hours
* Working Days: TBC prior to appointment.
* Shift Times: Negotiable: 7am – 3pm / 8pm - 4pm / 9-5pm with occasional late duties as and when needed to complete the role.
* One shift of a weekend is required [a Saturday or Sunday]
* Role Commencement: ASAP
