District Nurse with DNSPQ

Lincolnshire Community Health Services are seeking a full time District Nurse to join them within the Sleaford Community Nursing Team.
Applicants should hold the Community Specialist Practitioner qualification and have experience of managing a caseload of patients and leading a team of staff in delivering excellent patient care. Alternatively a Case Manager without the DNSPQ may be considered for the right candidate on a temporary basis with the expectation that the course is undertaken when available.
The service delivers care to patients in a variety of community settings across a 7 day service covering the hours of 8:00- 18:30 (shift work involved).

The District Nurse/Case Manager is essential to improving the quality of care that we deliver and in ensuring that the patient has a positive experience. Due to the rurality of Lincolnshire some areas are not well covered by public transport therefore having access to transport is essential.

- Current NMC registration
- Specialist Practitioner qualification in District nursing or similar
- Evidence of Continuous Professional Development
- Management or Leadership Qualification
- Teaching/mentoring qualification
- Experience of working in the NHS or related multi-professional setting dealing appropriately with key partner agencies
- Significant post registration experience in a relevant clinical setting
- Experience of working autonomously within a senior position
- Experience of leading on significant changes within a team
- Evidence of computer literacy and knowledge of Microsoft applications
- Evidence of extended clinical skills pertient to role
