Lincolnshire Community Health Services are seeking a full time District Nurse to join them within the Sleaford Community Nursing Team.
Applicants should hold the Community Specialist Practitioner qualification and have experience of managing a caseload of patients and leading a team of staff in delivering excellent patient care. Alternatively a Case Manager without the DNSPQ may be considered for the right candidate on a temporary basis with the expectation that the course is undertaken when available.
The service delivers care to patients in a variety of community settings across a 7 day service covering the hours of 8:00- 18:30 (shift work involved).
The District Nurse/Case Manager will hold a caseload of patients with varying health needs, coordinating care delivery. The District Nurse/Case Manager is not only integral to leading on patient care but leading the community nursing team through challenging times and the transformation of services, role modelling excellent behaviours which are in line with our trust values. The District Nurse/Case Manager is essential to improving the quality of care that we deliver and in ensuring that the patient has a positive experience.
