*Job title: Care Team Leader (Supported Living)*

*Hours: *Full time 40 hours per week and this role will include day, nights and sleep in shifts.

*Salary: *£12.80 per hour.

*Location:* The resident is currently in Howden, however we are sourcing a property at the moment and the resident will remain in North Tyneside. Drivers would be preferable for this role.

*About the Role*

Do you have previous experience working in care, leading a team and want to further your development within a Care Team Leader role which has fantastic career prospects?

This is an opportunity for a pro-active Care Team Leader to join an evolving Care Company. You will need to have previous experience of supporting people with learning disabilities, preferably in a supported living setting. In addition:
* *Have experience of supporting young adults with challenging behaviors is desirable*
* *Must have worked with a residential or supported living care setting previously.*
* *Must be physically well as the resident we support is very active and loves to be outdoors. *
* *Be able to lead a team and shift*
* *Hold a Level 3 in Health & Social Care or be willing to work towards this*
* *Having a driver's licence is desirable.*

The service is a single occupancy supporting 1-2-1 care for a resident who is transitioning from school to adulthood. The resident is very active with a great sense of humor.

*The role*
* Provide practical assistance and emotional support to empower their team in supporting the people they support to achieve their goals and aspirations.
* You will be proactive and support the development and skills of Support Workers. This will include recruitment, induction, supervisions, annual appraisals, training, coaching, rota management, sickness and absence management.
* Take the lead to help Support Workers understand their responsibilities and by doing so ensure that safe, effective, caring, responsive and well lead support and care is delivered at all times. You will complete evidence based supervisions and observation supervisions as well as supporting your team of support workers to complete competency based appraisals.
* Support the Service Manager/Deputy Manager in managing transitions into and away from the service. You will be able to liaise professionally with all outside professional agencies, and ensure that all information is shared appropriately. Ensure that all the people we support have person centered support/care plans. You will be able to create and update these plans as per any changes that are required.
* To oversee and support the development of excellent quality within the service. You will design, develop and maintain excellent service user records. Working in a supportive manner that empowers the service user to live their life in their chosen manner.

Experience:
* challenging behaviour and / or learning disabilities: 1 year (preferred)

* NVQ Level 2 or 3 in Health & Social Care (preferred)
