Remote Clinical Case Manager - Rehabilitation

This opportunity is working for a Rehab and Injury case management specialist company They are one of the UK's largest independent providers of rehabilitation case management services.

Typical patients would have sustained a Brain injury, Spinal cord injury or Major Trauma

Job locations Available: Yorkshire, Bedford, Scotland ( part time 3 days a week)

although this role is remote it will involve some field-based assessments and meetings to much be located in the areas above

Salary Package up to £45,000 + Bonus of Yearly £4,500 - £4,800 ( which will be paid Quarterly)

Monday - Friday 9 am - 5 pm

They also have these roles on a part time basis across 3 days a week if that suits better.

MUST BE A OCCUPATIONAL THERAPIST WITH , Neuro, Trauma, Brain Injury or Spinal injury and have dealt with patient rehab

Duties:

- Carrying out initial assessments

- Support Clients to identify & achieve full potential or focussed goal

- The Aim would be to facilitate a return to work or the best possible functionality

- Set out a road map for recovery

- Identify any potential barriers to recovery

- Point of Contact to client/patient

- working in conjunction with family members, employers, Medical professionals & Insurance organisations

- Implementing & Monitoring.

Must haves to apply:

· UK experience and registration with HCPC

· Right to work in the UK – They cannot offer working visas

· Must have own car with valid UK driver's license
