Job summary

We are delighted to offer this exciting opportunity for two appropriately experienced and registered health or social care professionals to work as part of the MND Care Coordinator Service that works across North Wales. The MND Care Coordinator role is responsible for co-ordination of care and support to people living with MND in North Wales.

The posts are based in the West (covering Mon, Gwynedd, Conwy and some of Denbighshire) or the East ( covering Denbighshire, Flintshire and Wrexham) The post holders will be normally expected to work Monday - Wednesday or Wednesday to Friday each week, during the hours of 9 - 5pm. providing cover though the week.

Applications are welcomed from professionals registered with any health or social care professional body, such as nursing, social work or the allied health professions.

Main duties of the job

The post holder will be expected to:

Deliver local support to patients diagnosed with MND in North Wales as a key point of contact. Track patients along their disease pathway from diagnosis to end of life. Coordinate the local multidisciplinary care of patients with MND across North Wales in primary and secondary and third sector care ensuring patients can access high quality care that adheres to the best practice guidance. Liaise with the Consultant Neurologists and the MND Care Centre in the Walton Centre NHS Foundation Trust at Liverpool for specialist input of MND care. To be a single point of contact for patients with MND, their carers and professionals as a resource for information advice and signposting.

The ability to speak Welsh is desirable for this post; English and/or Welsh speakers are equally welcome to apply.

Essential

Registration with professional body(nursing, relevant therapy body Physiotherapy , Speech and Language Therapy Occupational Therapy) Relevant 1st degree or evidence / working towards, supplemented by diploma level specialist training or experience

Experience

Essential

Significant post registration experience including experience relevant to palliative care and community working Broad range of community experience of speciality Experience of working in a multidisciplinary team Demonstrate leadership skills and experience

Desirable

Experience in caring for patients with progressive neurological conditions Working in the community setting Teaching skills and experience

Knowledge

Essential

Current services for MND patients in North Wales Advances and current issues in MND The research process. MNDA Standards of Care and national guidelines. Health care policy for neurological services

Skills

Essential

Have the ability to independently make clinical decisions. Able to manage work autonomously. Possess effective communication skills. Demonstrate a good understanding of audit and research and its application to / uses in practice. Able to influence and overcome resistance through application of specialist clinical knowledge and skills.
