Job summary

We have an exciting opportunity for a Clinical Pharmacist to join ourPrimary Care Network, covering four GP practices

in the southwest of Somerset. You will work in the Symphony HealthcareServices (SHS Medicines Management team alongside a team of pharmacists, pharmacy technicians, prescriptionclerks.

South Somerset West PCN works with primary care practices in Martock,South Petherton, Stoke Sub-Hamdon,Crewkerne, Somerton and Ilchester.

We offer full training and support from our established SHS team ofpharmacy technicians and pharmacists, and you will be given opportunities to develop your chosen career pathway.

You will provide Pharmacy services within Primary Care based within theSouth Somerset West Primary Care Network.

To support, promote and implement high quality, evidence-based,cost-effective, and safe use of medicines. This includes patient medicationreviews, helping practices achieve prescribing targets and supporting theimplementation of national and local guidance to improve patient care. You willneed to demonstrate ability to use initiative, have a positive attitude andapproach towards problem solving, and be a good team player.

The post availble is hours per week, working pattern to be discussed at interview.

£ - £ per hour with a NHS or Nest Pension offered.

Job responsibilities

JOB SUMMARY

To provide Pharmacy services within primary care based within the South Somerset West Primary Care Network, specifically working within the PCN Complex Care Team.

To support, promote and implement high quality, evidence-based, cost-effective and safe use of medicines. This includes patient medication reviews, helping the practice and Complex Care Team achieve prescribing targets and supporting the implementation of national and local guidance in order to improve patient care.

If not already a prescriber, to undertake training to become a Pharmacist Independent prescriber

MAIN DUTIES AND RESPONSIBILITIES

Provide clinical support support to the SSW PCN Complex Care Team to provide care to patients living at home with Multiple Long-Term Conditions (MLTCs) and leading on their medicines optimisation.

If required, provide sessional clinical support to the surgeries within the South Somerset West PCN, consulting with patients and leading on medicines optimisation.

If required, support the surgery to improve medicines reconciliation on discharge, formulary compliance, repeat prescribing management and medication reviews.

Support complex care patients, and those diagnosed with frailty syndrome through provision of structured medication reviews, prescribing support and input to multi-disciplinary teams.

Support Symphony and Somerset ICB plans to reduce medicines waste in primary care.

Interpret and provide complex medicines information to patients, carers and healthcare professionals.

To provide information, advice and guidance on the safe, rational, effective and economical use of medicines.

Undertake clinical audit and other outcome measures to support patient care and safety.

Create and implement medicine related guidelines, policies and SOPs as appropriate to the different service areas.

Provide and receive complex, sensitive information and communication relating to pharmaceutical care of patients.

Provide prescribing service within sphere of competency to patients in primary care, as pharmacist independent prescriber.

To develop, advise, agree and implement action plans at PCN level, monitor the effect of changes made and ensure that any benefits are maintained in the longer term. This will include:

Reviewing prescribing data for practices to highlight areas where there is scope for improving the clinical effectiveness of prescribing. Obtaining agreement with practices to implement medicines optimisation projects with the overall aim of following national prescribing guidance and cost effective prescribing. Advising GPs and other health professionals on cost effective prescribing and aiding rationalisation of drug choice via formulary/guideline/protocol development.

Record clinical activity, interventions, KPIs and to monitor the outcome of interventions.

Provide training on medicines on a regular basis to GP surgery staff, patients and carers in all relevant aspects of medicine use.

To maintain knowledge of developments in pharmacy practice on a regular basis in line with General Pharmaceutical Council (GPhC) CPD requirements

To fulfil the professional requirements of the Code of Ethics and Standards of Good professional Practice of the GPhC

The post holder will be responsible to the SHS Lead Pharmacist and Practice Clinical Leads, but will be expected to use their own initiative to make complex decisions within defined parameters with little supervision.

To undertake training as a Pharmacist Independent Prescriber if not already qualified as such.

Qualifications

Essential

- GPhC registration as a Pharmacist - Certificate or MSc/Diploma Pharmacy Practice (or equivalent experience)

- Independent prescribing qualification

- At least 2 years post-registration pharmacy experience - Clinical Pharmacy experience - Clinical audit experience - Knowledge and understanding of Primary Care

- Previously experience of working within integrated healthcare organisations or teams
