We are looking torecruit an enthusiastic, compassionate and motivated staff nurse with excellentcommunication skills to join our dynamic nursing team. Dedicated and modern working environment by delivering the best compassionateand clinical care standards.

We are looking for anoutstanding individual who will make a real contribution to palliative care.You will need to be a first level nurse with at least 12 months postregistration experience. You willprovide a holistic model of care to facilitate the delivery of high standardsfor patients who have specialist palliative care needs. Palliative careexperience is desirable but not essential.

Successful candidateswill have the opportunity to gain enhanced clinical and management experienceto support career progression in a specialist palliative care environment, andwill have the opportunity to attend in-house training programmes.

Main duties of the job

Provideholistic nursing care to patients in the In Patient Centre to ensure that thedignity, safety and confidentiality of patients is respected at all times andthat all patients receive the highest standard of physical, psychological,spiritual and emotional care.

Todevelop and deliver high quality nursing care for patients who are admitted tothe in-patient centre, reflecting the St Lukes philosophy and ethos.

Actas a role model and mentor to new and junior staff.

Tobe responsible for developing self-awareness through reflective practice,clinical supervision and to take part in annual review of personal andprofessional development plans.

Job responsibilities

1 Clinical Responsibilities

Underthe supervision of the Nurse in Charge to be responsible for undertaking acomprehensive nursing assessment to formulate, deliver and evaluate care plans.

Toprovide support, information and advice to patients, families and professionalcolleagues

Towork collaboratively with the multi-disciplinary team (MDT)in formulating care plans for the patients in their teams, this will include MDT meetings

Maintainaccurate, current, comprehensive and concise documentation for all patients inaccordance with legal, professional and hospice requirements/guidelines.

Toensure information is inputted into the Infoflex patient database system on adaily basis.

Takeprofessional accountability for ensuring the safe and appropriateadministration of medicines to patients, adhering to standards defined by theNursing & Midwifery Council and St Lukes Medicines Code.

Toensure and deliver the effective communication is maintained at the highestlevel of interpersonal and advanced communication skills with patients, carersand staff in order to provide and receive complex and sensitive information inemotive situations.

Todemonstrate an understanding of Clinical Governance and Risk Management andapply to work situation.

Tomaintain custody and storage of controlled and other drugs, checking andwitnessing administration process ad strictly adhering to the St LukesMedicines Code.

Totake responsibility for day to day monitoring of risks, incidents within IPC tominimise risk to patients, visitors and staff.

Toprioritise and time manage in order to meet the unpredictable and on occasions conflictingneeds of the service.

Keepabreast of modern developments in clinical practice. To attend trainingprogrammes/educational courses as agreed between you and your line manager andemployer.

Tobe responsible for the safe use and storage of specialised equipment; reportingany deficiencies as stated within the Hospice policies.

2 Managerial/Developmental Responsibilities

Towork closely with the Head of IPC Nursing Services, IPC Senior Sister andsenior nursing team to provide support in developing, implementing and evaluatingservices.

Maintaina sensitive and caring atmosphere for patients, carers and staff and volunteerswithin the In-patient centre by example and professionalism.

Todemonstrate leadership skills.

Tosupervise the work of unqualified and new members of the team.

Todevelop and discuss new initiatives for the development of the department inaccordance with the standards of the Healthcare Commission, NICE guidelines andSt Lukes Service Improvement Project.

Tomaintain, develop and strengthen links with the Active Intervention Centre,Specialist Palliative Care Community team and Primary Health Care Team.

Ensureeffective use of the resources available to the team and the financialimplications associated with these for service delivery and futuredevelopments.

Takeresponsibility for the management of the unit in the role of Nurse in Charge inthe absence of the Senior Nursing Team.

Providegood public relations with professional and non-professional visitors to StLukes.

3. Teaching and Education

Toclearly convey complex information through spontaneous and planned advice,teaching and instruction to relatives, carers and other professionals.

Topromote understanding of the aims of the In Patient Centre care and to ensure aconsistent approach to patient care.

Demonstratethrough evidence based practice both clinical and leadership skills, promotingsharing of knowledge and experience amongst nursing and medical staff.

Tokeep abreast of current research developments in palliative care and anyimplications they may have to clinical practice.

Toparticipate in the induction, support and mentorship of new staff and provideteaching for/to Junior Staff.

Assistin fostering an atmosphere of reflective practice where critical incidents area learning opportunity for all.

Participatein team meetings, acknowledge the contribution of all team members as valuableto patient care and St Lukes team.

4 Audit and Research

Toparticipate in audit, research and identify evidence based practicerequirements, which will enhance the delivery of palliative care services.

Toundertake the evaluation of work and current practices through the use ofprojects, audit and outcome measures, either individually or as part of aresearch team. Make recommendations for changes to practice that may impact onservice delivery, business planning or strategic reviews.
