Job summary

This is a exciting and fantastic opportunity to work within Brent, NW London, with a large London PCN comprising 10 GP practices, delivering care for 77,000 patients.

Following the release ofARRS GP information and funding we are looking for newly qualified (within 2years and not having a substantive post), enthusiastic forward thinking GP(s)to join Harness South PCN on a fixed term salaried contract, preferably with an immediate start date.

Main duties of the job

The postholder's role will primarily involve patient-facing clinical work supporting the PCN GP practices alongside a focus on PCN initiatives including novel, innovative at-scale work and opportunities. Additionally, they will offer support to the GP Enhanced Access Hub and care homes within the PCN, providing enhanced services to the registered population of the Harness Federation. The Harness Federation consists of two Primary Care Networks (PCNs), each comprising 10 GP practices.

The post holder will demonstrate courageous and critical thinking and is a caring, compassionate and committed experienced general practitioner who, acting within their professional boundaries, will provide care for the presenting patient from initial history taking, clinical assessment, diagnosis, treatment and evaluation of care. They will demonstrate safe, competent clinical decision-making and expert care, including assessment and diagnostic skills, for patients within the general practice.

The post holder will commit to demonstrating critical thinking in the safe clinical decision-making process. They will communicate and work collaboratively with the general practice team to meet the needs of patients.

About us

The PCN consists of 10 GP practices based over 9 sites across Brent, caring for a population of over 77,000 patients.

We are a highly supportive PCN and the GP practices all work closely within the PCN, understanding the importance of a good work/life balance with an open-door ethos.

We work together with a range of local providers, including acute trusts, community services, social care, voluntary sector and patient participation groups, to offer more personalised, co-ordinated health and social care to our local population.

Job responsibilities

Clinical responsibilities

Assess and triage patients, including same day triage, and as appropriate provide definitive treatment (including prescribing medications following policy, NICE (national) and local clinical guidelines and local care pathways) or make necessary referrals to other members of the primary care team Implement and evaluate treatment/interventions and care for patients presenting with an undifferentiated diagnosis Clinically examine and assess patient needs including history taking, assess, diagnosis, plan, implement and evaluate interventions/treatments for patients with complex needs and onward refer where appropriate Deliver high-quality healthcare to care home residents who often have complex medical needs, conducting thorough initial assessments upon admission to the care home Use complex decision making to inform the diagnosis, investigation, complete management of episodes of care within a broad scope of practice Prescribe and review medication for therapeutic effectiveness, appropriate to patient needs and in accordance with evidence-based practice and national and practice protocols, and within scope of practice Assess, identify, and refer patients presenting with mental health needs in accordance with the NSF for Mental Health Implement and participate in vaccination and immunisation programmes for both adults and children Support the delivery of advanced care plans Actively take a personalised care approach and population centred care approach to enable shared decision making with the presenting person Recognise and work within your own competence and in accordance with professional codes of conduct, maintaining accurate and contemporaneous health records Be aware of your duties and responsibilities regarding current legislation and adhere to our policies and procedures on Safeguarding Children and Safeguarding Adults.

Experience

Experience of working in a primary care environment Experience of dealing with a range of clinical conditions Experience of continued professional development Proven ability to evaluate the safety and effectiveness of own clinical practice Understanding of evidence-based practice Evidence of working autonomously, with some support and supervision
