Job summary

Are you a dynamic, innovative nurse interested in developing your skills in Community Stroke Care? Would you like support from an established Stroke Nurse Team as well as an integrated multi-professional team to progress your skills and achieve excellence for your patients?

This is an exciting opportunity for a permanent Band 6 Stroke Nurse (18 hours a week with regular shifts), within the East Berkshire Earlier Supported Discharge team for Stroke Rehabilitation. The post has an office base at St Mark's Hospital, Maidenhead, clinical care will be provided in patients' own homes within East Berkshire but some cover to the West of Berkshire will also be required. The successful candidate must be a car driver.

In this diverse role the successful candidate will deliver high quality nursing care for patients who have been discharged from hospital following a stroke, related to their individual goals and needs. The post holder will receive support for their caseload from other experienced Band 6 Nurses as well as from the Band 7 Stroke Nurse Team Lead. The post holder will be able to work in a cohesive manner with and seek support from other members of the specialist team, Occupational Therapists, Physiotherapists, Speech and Language Therapists, Psychologists and Therapy team is forward thinking and there is a culture of continual improvement to achieve exceptional outcomes for patients.

Main duties of the job

You will work closely with other Nurses to complete holistic nursing assessments as well as plan, deliver and evaluate evidence-based interventions to achieve the best outcome for patients.

Develop knowledge and clinical competence within stroke and rehabilitation and utilise this to deliver excellent clinical care with the support of Senior Nurses and the Multi-disciplinary Team.

Use excellent communication skills to engage and motivate Stroke Survivors to engage in rehabilitation and adhere to secondary Stroke prevention advice, as well as liaise with relevant professionals within the team and in wider services such as GP practices, Stroke Consultants and the Stroke Association

To complete relevant administrative duties such as accurate and concise documentation of notes, reports and data recording as required by the role.

You will play a role in co-ordinating referrals establishing suitability for the service and attending hospital multidisciplinary meetings as required.
