Job summary

An exciting opportunity has arisen for the right candidate to join the Community Home Oxygen Service at the Sheffield Children's Foundation Trust.

The team member will be required to visit patients in a variety of settings including community and hospital (inpatient and outpatient clinics).

Main duties of the job

As part of a senior nursing team, help co-ordinate the specialist service in a variety of settings, ensuring the quality-of-care provision to children.

To assist in providing specialist education, training and information to children, families and members of multidisciplinary team.

To be responsible for monitoring patient condition, administering treatment and communicating change to senior specialist nurse or multidisciplinary team.

To be responsible for nursing practice development.

Job description

Job responsibilities

Clinical Practice

Undertake assessment of needs and negotiate appropriate care plans to meet the individual needs of children and their families

Deliver expert, evidence based care to children and families who require long term home oxygen therapy.

Assist in the discharge planning for children, liaising with primary care and ensuring appropriate follow-up

Provide planned and unplanned, and urgent visits as required.

Provide specialist telephone advice service to patients and families, and serve as a point of contact for other professionals

Communicate and liaise with multidisciplinary teams in both the hospital and community and with other agencies such as education, social services and voluntary agencies to promote a seamless service

Communicate effectively to identify and address frequent issues of non-compliance/ barriers to communication and potential child protection issues

Provide cross cover for other team members

Education And Training

Provide education, training and specialist advice for patients, their families

Act as a specialist resource for ward staff, other professionals and agencies

Participate in provision of education, update and training sessions as necessary

Ensure that opportunities are utilised to promote health education to patients and staff

Help to develop information and resources for patients, parents and other professionals

Managerial

Line manager to Band 5 nurse

Ensure the views of all patients are effectively sought, channelled and acted upon, including the efficient actioning of complaints in accordance with relevant trust policies

Contribute to service development and business planning processes

Be responsible for the maintenance and accuracy of relevant information systems/databases

Feedback any changes in patients conditions to multi-disciplinary team

Professional

Participate in benchmarking, audit, development and implementation of systems for measuring and assessing quality.

Contribute to service development, audit and research activity to improve quality of care.

Inform team leaders on issues concerning professional practice

Utilise opportunities to develop knowledge and expertise regarding chronic lung and other respiratory conditions.

Develop local and national networks with professional groups in order to improve knowledge, expertise and patient care.

Person Specification

Qualifications and Training

Essential

RN Child (RSCN) or equivalent 998 or equivalent teaching certificate

Desirable

Diploma level post basic nursing qualification in relevant field Degree in relevant subject orking towards completion of nonmedical prescribing

Experience

Essential

Significant post registration experience Completed Band 5 co-ordinator competencies (or equivalent)

Desirable

Experience of working with the Specialty patient group Teaching and mentorship qualification Audit / Research experience Project or caseload management
