Job summary

Innovations in Care provides comprehensive care and support to people living with complex mental health diagnoses within their own homes.

We are seeking an experienced Mental Health Nurse to join our award winning team in the role of Care Coordinator. The Care Coordinator will work with our staff and clients to ensure care packages reflect current best practice and promote long term development & growth.

This position is offered equivalent to Agenda for Change Band 6/7 dependant on experience. The successful candidate will be joining the team at an exciting time and will develop this role to suit their skills and experience.

Main duties of the job

We are seeking an experienced and dedicated Mental Health Nurse to join our Adult Supported Living Service in Kingston upon Hull. The ideal candidate will have significant experience working with and coordinating care for patients with complex emotional needs. They must be registered as a Mental Health Nurse in the UK and have a comprehensive understanding of personality disorders. Knowledge and experience in working with personality disorders are essential. This role involves providing clinical leadership to support staff, mentoring and training junior staff, and developing therapeutic relationships with our service users. The Care Coordinator will work closely with local authorities, primary and secondary care, and community mental health services to develop and maintain comprehensive care packages to support individuals living in their own homes.

Job description

Job responsibilities

Develop individualized care plans in collaboration withpatients, families, and the multidisciplinary team.

Have an understanding ofevidence-based therapeutic interventions, to support our clients to engage in these processes with their care team

In hours, provide immediate support and intervention, utilising de-escalation techniques to manage acute episodes of mental health crisis

Provide training, supervision and mentorship to junior staff to equip them with the tools and knowledge to manage acute episodes of mental health crisis

Develop and implement crisis management plans, behaviour response plans and community access guidelines in collaboration with the wider multi disciplinary tam

Build therapeutic relationships with clients to facilitatetrust and effective development

Provide clinical leadership and supervision to supportstaff, ensuring high standards of care.

Deliver training sessions to support staff on mental healthissues, treatment techniques, and care strategies.

Promote a culture of continuous learning and professionaldevelopment within the team.

Coordinate care plans and services with local authorities,primary and secondary care providers, and community mental health services.

Ensure seamless communication and collaboration among allstakeholders involved in client care.

Participate in multidisciplinary team meetings, caseconferences, and treatment planning sessions.

Person Specification

Knowledge and Skills

Essential

Comprehensive understanding of the Mental Capacity and Mental Health Acts and their application in community settings Excellent communication and interpersonal skills. Ability to work effectively in a multidisciplinary team. Proficiency in electronic health record (EHR) systems.

Desirable

Crisis intervention and conflict resolution skills. Expertise in a variety of therapeutic techniques. Confidence managing your own workload Significant clinical experience in assessing, planning, implementing and evaluating care packages

Experience

Essential

Significant experience working within an acute mental health setting at band 6 or above Extensive knowledge and experience working with people diagnosed with personality disorders Experience of proactively managing your own case load

Desirable

Experience of working within the Mental Health Crisis Intervention Team

Qualifications

Essential

BSc(Hons) Nursing (Mental Health) or equivalent pre-registration qualification Management / Leadership qualification or equivalent experience Mentoring/Clinical Supervision training at level 6 or above (or willing to complete) Teaching/Training Qualification (or willing to complete)

Desirable

Post registration qualification (or willingness to complete) Completed Knowledge & Understanding Framework Training
