Job summary

We are looking for a social worker assistant who has experience of working with adults and their families. You will be providing psychosocial and practical support as necessary, to patients and family members.

Main duties of the job

To monitor referrals and complete day-to-day tasks as allocated by the Senior Social Worker. To manage own caseload, with the support of the line manager. Help patients, their carers, and families to establish their needs, and support them to understand how they may access services that can meet their need. Advocate for patients and carers as needed with other statutory and voluntary agencies. One-to-one interviews and direct work either assessing needs and risk and / or providing emotional support for patients and family carers. This work can take place on site or potentially in patients' homes. To give basic benefits advice and make referrals to specialist benefits advisors as required. To offer social support and pre bereavement support to patients and their carers. When in direct contact with patients and carers, offer support, guidance, signposting and referral to services if required including referral to bereavement services after the death of a patient. To promote an understanding within the Multi-Disciplinary Team of the social difficulties encountered by patients and their carers to influence the development of their care plans. Maintain confidentiality and good professional practice, to ensure that there is the fullest co-operation with external agencies in the patients' best interests. To ensure carers of adult palliative care patients are provided with the opportunity to have their needs assessed using the Zarit tool.

Job responsibilities

Contribute to the IPU MDT and care planning sessions. To support the carers support sessions and provide individual support and advice to carers as required. You'll be working closely with the RGP social worker, outpatients and IPU team and benefits advice service. Attend daily planning sessions with the RGP social worker. Attend relevant meetings as appropriate, supporting the role of the social worker within the MDT. To work closely with the senior social worker and the Supportive Care team. To build good relationships with Social Services staff in Hertfordshire and Buckinghamshire. To become familiar with carer support available and promote links with the families we service. To become familiar with a range of benefits advice and grants available for a wide range of diagnoses. Person Specification

Experience

Essential

Experience working as a patients' advocate. Experience of networking and building cross boundary relationships.

Desirable

Experience of leading and motivating teams of volunteers.

Additional Criteria

Essential

Excellent listening and communication skills both written and oral. Understanding the social model of care. Basic understanding of palliative care and loss and bereavement. Understanding the role of a multidisciplinary team and the ability to build high level working relationships. A good understanding of technology with a working knowledge of Microsoft Office packages Word, Excel and Outlook. Understands the importance of respecting confidentiality and gaining consent. Good administrative and record keeping skills and able to work with good attention to detail. Strong organisational skills. Sensitivity and understanding to the needs of the volunteers and customers, many of whom may have experienced Rennie Grove Peace services or be bereaved themselves. Desirable

Understanding the role of the hospice. Understanding the issues relating to the safeguarding of vulnerable adults. Knowledge of mental capacity. Ability to deal with difficult emotional situations. Strong problem-solving skills and able to implement new initiatives.
