Sandwell Community Learning Disability Team has a vacancy for a Band 6 Community Learning Disabilities Nurse.  You will be part of an experienced and knowledgeable nursing team team with support from the MDT. The post is flexible over hours and days to be tailored to individual needs while meeting the needs of the service. However, it is predominantly at Monday-Friday service.  The post would suit an experienced nurse seeking a new challenge or a newly qualified nurse as a developmental role. We provide specialist support, advice and guidance to individuals, families and carers. You will have the opportunity for continuing professional and career development. 

Main duties of the job

To provide specialist health support, advice and guidance to individuals, families and carers. To provide specialist support as part of the multi-disciplinary team. To complete specialist health assessments as clinically indicated. To participate in referral meeting, team meetings and wider MDT meetings. To contribute to team and service development. To carry an individual caseload. To keep up-to-date with research and development in your fields of practice. To attend training relevant to the role. To maintain NMC registration and remain up-to-date with research and development in learning disability nursing practice. To complete individual risk assessments under supervision and to put a safety plan in place if needed. To communicate with and support social workers, care providers and other agencies and services. To supervise and assess student nurses on placement with the team.

Detailed job description and main responsibilities

To practice within the NMC Code of Professional Conduct. To be professionally accountable for the effective and efficient management of a defined caseload, taking into account the need to combine your own responsibilities and those of the multi-disciplinary team. To be responsible for setting high standards of care within the resources available and to review and monitor these on a continuous basis through the process of audit, case review and on-going supervision. To promote a positive approach to health education and health promotion. Participate in consultancy and specialist input to a range of other professionals and agencies in relation to individuals with complex needs. Advise and influence local and regional policy, strategy and operational practice in relation to health needs of people with learning disabilities.

Management

To undertake and participate in performance reviews on an annual basis ensuring feedback on performance is given and received at regular intervals throughout the year. To promote the principals of Improving Working Lives amongst the team ensuring fairness and equity. Communicate effectively with your team on a timely basis ensuring robust systems exist. To display, promote and monitor high standards of organisational skills, planning and priority setting both clinically and organisationally. To monitor your clinical caseload, including care plans, case reviews and participates in any relevant meetings. To maintain, as required, record keeping within NMC and Trust Guidelines and provide clinical activity information as required. The post holder should also be cognisant of the communication of confidential information in line with the Data Protection Act and Caldicott Guidelines. To contribute to the induction of other members as relevant. To support the work place for both pre-registered and post registered student nurses. To ensure work place audits are maintained and supported. To comply with the Health and Safety at Work and fire regulations in the work place.

Clinical

To work pro-actively with all members of the community nursing, PAMHS and Behaviour support team; as well as the wider multi-disciplinary team. To carry a caseload, which requires the provision of specialist assessments; formulation; implementation and evaluation of measurable interventions. To ensure these are supported by robust risk assessments and detailed support plans. To maintain clear and contemporaneous records of all case work, using approved electronic recording systems. Maintain electronic diary. To produce formal reports and feedback outcomes to a range of stakeholders whilst maintaining appropriate levels of confidentiality. To work within legal parameters, having a working knowledge of relevant legislation. To effectively communicate with a range of stakeholders across a range of settings. To work within the guidance of the Trust/local ‘Lone Working’ policy, using agreed ‘report & confirm’ systems. To manage own caseload effectively under the supervision of the clinical lead, highlighting any concerns to the MDT without delay. To actively participate in regular clinical supervision with the team manager. To participate in own appraisal and ensure objectives and work plans are met. To comply with all Mandatory and Statutory training requirements. To provide support and mentorship to student nurses and other learners. To participate in audit and service evaluation programmes as required. To undertake any other duties of a similar nature consistent with the responsibilities of this post in order to provide a quality learning disability service. To maintain up-to-date knowledge of development within the sphere of learning disabilities and maintain NMC registration, adhering to the Councils code of Professional Conduct. To ensure knowledge of safeguarding Vulnerable Adult and take action to safeguard children, young people and adults where necessary. To cover workload of team members in their absence. Develop effective working relationships with professionals both internal and external to the service.
