Job summary

We are looking for a Paramedic to work in our community hospital at home team. This is an exciting new opportunity to work within our MDT team visiting patients in their own home, giving hospital level care in their home environment.

We provide hospital at home care in the community, working alongside our GPs, therapists, Nursing staff and specialist post is working within Hertfordshire Community Trust, in Upper Lea Valley Locality.

In addition to the basic salary, this post attracts a High Cost Area Supplement of 5% of basic salary subject to a maximum payment of £2,011 per annum (pro rata where applicable).

You must hold a full valid driving licence and access to a car to use regularly for business purposes - including business insurance - is essential (unless you have a disability as defined by the Equality Act 2010).

Main duties of the job

The Upper Lea Valley Integrated Community Team has an opportunity for a full time band 6 paramedic to join our need a dynamic and motivated paramedic to support the team to ensure patients receive high value healthcare. The post holder must have excellent communication skills and demonstrate collaborative working.

The team comprises of nurses, physiotherapists, OT's and community matrons, all working to provide evidence based nursing care to patients in their own homes. The service operates 8am - 8pm, 365 days a year.

You will need to demonstrate an understanding of current professional and community issues. You must have commitment and flexibility in order to be innovative and responsive to continual developments within the community. In return for this we offer you professional development in both clinical and leadership programme.

A full valid driving licence and access to a car to use regularly for business purposes is essential (unless you have a disability as defined by the Equality Act 2010).

Person Specification

Qualifications and Training

Essential

Registered Nurse Mentor/Preceptor/998 or equivalent Specialist Nursing Practice(District Nursing) Degree or will undertake training Nurse Prescribing or will undertake training

Experience and Knowledge

Essential

Knowledge of recent developments in NHS Knowledge of Evidence based practice Evidence of personal development Knowledge of NMC code of professional conduct Experience at Band 5 Community Nurse level Management of a team Management of change within a complex environment Experience of teaching others
