Job summary

The PCMH team is based across several sites in Sheffield offering care for those service users whose care does not meet the threshold for secondary care and their care can safely be managed in a primary care setting. The service has been up and running successfully for more than 2 years and provides mental health services to the 15 PCNs (Primary Care Networks) across Sheffield. Each PCN being a cluster of GP Surgeries in a locality.

The PCMHT team is made up of mental health practitioners, peer support workers, recovery workers, psychiatrists, health coaches, pharmacists, psychology and occupational therapists, community connectors, and administrative staff. There are plans to develop exciting new roles to complement the medical support within the team for example GPs with Special Interest in mental health in PCMHT This novel team of clinical and non-clinical staff together will provide care to people of Sheffield who have a Serious Mental Illness (SMI) and can be managed safely in primary care locations close to their homes.

Service users referred by GPs or primary care colleagues those who have a SMI and meet the criteria for PCMHT rather than CMHT. The new integrated model of services will allow for primary care clinicians to work in a truly collaborative manner with PCMHT and CMHT colleagues.

Main duties of the job

Provide comprehensive psychiatric assessment and formulation to inform holistic management plans-face to face and or remotely. Assessment and management of risks. Assessment and management of physical ill-health Medication management including prescribing, monitoring, and stabilisation of medications as per NICE. Provide support and advice to MDT and wider team in delivering expert, safe and effective care. Provide training and formal supervision to medical trainees and non- medical staff. Mutual consultation with GPs, Pharmacists, ACPs and other roles. Provide systemic leadership in building relationships with colleagues in primary care, VCSEs and external agencies. Provide leadership in development of culture of continuous learning and quality improvement. Provide leadership in reviewing policies and governance structures.

Qualifications

Essential

Certificate of Completion of Training in General Adult Psychiatry (ST6 trainees should be within six months of attainment at the time of the interview) or CESR equivalent. Inclusion on the Specialist Register or eligibility for inclusion Section 12 (2) MHA 1983 approved.

Desirable

MRCPsych or equivalent MD, Other professional qualification (, MSc)

Knowledge

Essential

Supporting flow through working effectively with primary care service and VCSEs to enable full independent recovery of our service users. Ability to supervise members of the team, especially medical roles, pharmacists, and wider MDT members. Ability to retain the unique clinical role as a consultant psychiatrist while navigating the complexity of providing liaison style care and a large supervisory role.

Experience

Essential

Experience in General Adult Psychiatry Experience to a level allowing recognition of trainer status by the Royal College of Psychiatrist. Experience in teaching undergraduates & postgraduates. Evidence of experience in medical audit and understanding of principles of clinical governance Practical experience of Clinical Audit Able to use and appraise clinical evidence.

Desirable

Teaching/presentation experience (, conference presentation, EBM, research experience).

Ability to exercise systemic leadership skills and in a newly formed multi-disciplinary team. Understanding of importance of population health approaches and health inequalities Understanding of and ability to deliver care in collaboration with primary care colleagues as well colleagues in voluntary care sector.
