JobPurpose:

  • Assistin delivering the Council s statutory responsibility for adultassessments focusing on prevention and wellbeing as per the CareAct (2014).
  • Implement interventionsto prevent the escalation of needs and maintainindependence.
  • Recommend demonstrateand provide equipment and housingadaptations.
  • Work therapeuticallywith adults to addressproblems.

MajorTasks:

  • Manage acaseload and conduct assessments including complex cases andhousing adaptations.
  • Identify andmitigate risks develop care plans and collaborate with families andpartner agencies.
  • ProvideOccupational Therapy interventions to delay or reduceneeds.
  • Participate in group meetingssupport professional development and contribute totraining.
  • Supervise and delegatetasks to unqualifiedworkers
  • Ensurequality and costeffective servicedelivery.
  • Engage in preventativework and support informed decisionmaking by adults families andcarers.
  • Maintaincase notes write reports and promote equality and safety in alltasks.
  • Stay updated on OccupationalTherapy issues legislation and evidencebasedpractices.


Requirements

Qualifications:

  • HCPCregisteredOccupational Therapist.
  • Commitmentto professional development as per HCPCguidelines.

Experience:

  • Experiencein adult and older people s Occupational Therapy includingassessment prevention andintervention.
  • Decisionmaking skillsin care issues involving diverse culturalbackgrounds.
  • Knowledge of the CareAct Mental Capacity Act and Mental HealthAct.
  • Understanding of currentgovernment policies and legislation on housing for adults and olderpeople.


Qualified Social Worker with HCPC registration. Minimum one year spost-qualification experience. Strong assessment, planning, andintervention skills. Excellent record-keeping and report-writingabilities. Proactive, collaborative, and able to manage a complexworkload.
