An exciting opportunity has arisen to join our District Nursing area team in south meirionnydd to work in collaboration with the Community Resource Teams in providing care closer to home.

We believe in developmental and training opportunities for our staff and therefore are currently seeking to recruit highly motivated, passionate Band Community Nurses who are keen to develop and work towards one of the following roles

Band District Nurse by undertaking the District Nurse Qualification.

Chronic Condition Case Manager by undertaking disease specific training to provide evidence based care to those individuals with highly complex needs.

Nurse Practitioners by undertaking a modular approach to attain the clinical skills required.

The successful applicant will need to have excellent communication skills with an ability to provide clinically effective care within the District Nurse Team. The West Division of Betsi Cadwaladr University Health Board delivers District Nursing Services across the Counties of Gwynedd and Anglesey working in an integrated way with partner organisations to deliver high quality evidence based care in order to maintain patients in their own homes and communities, averting hospital admissions, empowering the patient towards self management and implementing seamless patient centred care and treatment.
You must be able to travel between patients homes both within towns and rural areas in a timely manner.

The successful applicants will be based at Bala District Nursing Team- South Meirionnydd, and will support the teams in managing a Single Point of Contact for all District Nursing Referrals.

This post is rotational over hours. day & night.

The ability to speak Welsh is desirable for this post; Welsh and/or English speakers are equally welcome to apply.
