Job description

Job responsibilities

To support the GP practice from within to develop the clinical pharmacist role under the guidance of the Lead Pharmacist. To support, promote and implement high quality, evidence-based, cost-effective and safe use of medicines. This is a patient facing clinical role that will include medication reviews, medicines optimisation and reconciliation and working as part of a team to achieve prescribing targets as well as supporting the implementation of national and local guidance in order to improve patient care. To act as a specialist resource colleague to the GP Practice and as a team member to the practice pharmacy team, to answer medicines-related queries, to signpost to other services and to liaise with other healthcare professionals, including secondary care when required. To support development of projects within the practice. Undertake clinical medication reviews with patients with multiple health issues and implement own prescribing changes (as an independent prescriber) and order relevant monitoring tests. Manage own caseload of patient with medication issues, high risk patients and housebound and care home patients. Undertake clinical medication reviews for patients at increased risk of hospital admission and harm from poor use of medicines. Implement own prescribing changes (as an independent prescriber) and order relevant monitoring tests. See patients in the clinic setting and, in partnership with primary healthcare colleagues, implement improvements to patients medicines, including stopping medication as appropriate. Support patients with long-term conditions and prescribe as appropriate (as an independent prescriber) within own area of clinical competence. Answer and record complex and potentially sensitive queries about medicines or NHS systems from healthcare professionals and from members of the public using professional judgement where guidance is unavailable. Deal with queries, or complaints, from patients, who may be distressed or unhappy with changes made to their medication and support the wider complaints / governance system. Repeat Prescribing. Support pharmacist colleagues and the practice to produce, implement and audit repeat prescribing policies and processes. Management of the repeat prescribing reauthorisation process by reviewing patient requests for repeat prescriptions and reviewing medicines reaching review dates; make necessary changes as an independent prescriber, and ensure patients are booked in for necessary monitoring tests where required. Management of Medication on Discharge Reconcile medicines following discharge from hospitals, intermediate care and into care homes; identify and rectify unexplained changes; manage changes without referral to the GP; perform clinical medication review; produce post-discharge medicines care plan and work with patients and community pharmacists to ensure patients receive the medicines they need. Work in partnership with hospital colleagues to proactively manage patients at high risk of medicine-related problems before they are discharged to ensure continuity of care. Identify cohorts of patients at high risk of harm from medicines. Working with patients and the primary care team to minimise risks through medicines optimisation. Share practice searches to identify cohorts of patients most likely to be at risk of an unplanned admission and readmissions from medicines. Work with case managers, multidisciplinary (health and social care) review teams, hospital colleagues and virtual ward teams to manage medicines-related risk for readmission and patient harm. Put in place changes to reduce the prescribing of these medicines to high-risk patient groups. Support and contribute to the development of the pharmacy team through shared learning, regular meetings and updates. Support input into regular role reviews with the Lead Pharmacist. Identify and provide leadership on areas of prescribing requiring improvement. Conduct own audits and improvement projects. Guide and supervise the audit activity within the pharmacy team. Work to agree a work plan and key areas of focus, identifying areas for development and requiring additional clinical support.
