You will contribute to an exciting programme of transformation as we deliver our ambition to provide an improved person-centred, co-ordinated patient care system. This role will be solely practice-based, across our network and will offer the post-holder the opportunity to work with the Directed Enhanced Service contract for PCNs, which provides new and exciting opportunities for experienced clinical pharmacists to work.

Main duties of the job
Rugby Health are currently looking for an experienced clinical pharmacist, ideally IP qualified, to provide primary support to general practice staff with regards to prescription and medical queries, supporting with repeat prescriptions system, acute prescription requests and medicines reconciliation on transfer of care. The post-holder will work as part of a multi-disciplinary team in a patient-facing role. The post-holder will take responsibility for areas of chronic disease management within the practices and undertake clinical medication reviews to proactively manage patients with complex polypharmacy, especially for other people, people in residential care homes and those with multiple co-morbidities. The post-holder will work independently but closely with the lead clinical pharmacist, GP lead prescriber, CCG MO team, community and hospital pharmacy colleagues.

Job responsibilities
Manage caseload of patients with common/minor/self-limiting ailments while working within the scope of practice, formalised training and limits to competence to support all treatment and preventative healthcare services within the PCN, as delegated and agreed by the Operations Director. Integrate with community and hospital pharmacy to help utilise skill mix, improve patient outcomes, ensure better access to healthcare and help manage workload. Undertake clinical medication reviews with patients with multi-morbidity and polypharmacy and order relevant monitoring tests according to protocols and policies. Review the on-going need for each medicine, review monitoring needs and support patients with their medicines taking ensuring they get the best use of their medicines (medicines optimisation). Review the use of medicines most associated with unplanned hospital admissions and re-admissions. Suggest and recommend medicines management solutions to reduce the risk to identified patients and follow outcomes. Prescribe safe, appropriately and within limits of competence and independent prescribing Qualification Provide telephone help line for patients with questions, queries and concerns about their medicines with some bookable appointments. Work with case managers, multidisciplinary (health and social care) review teams and hospital colleagues to management medicines-related risk for re-admission and patient harm. Participate in clinical audits, presenting results and providing recommendations to improve projects Undertake clinical audits of prescribing in areas directed by PCN, feedback the results and implement changes in conjunction with the multi-disciplinary teams. Lead on evidence based medicine and support CCG medicine strategies. Adhere to national and local policy and guidance that affects patient safety through the use of medicines using MHRA alerts, product withdrawals and emerging evidence from clinical trials. Ensure prescribing is in accordance with APC guidelines and monitor prescribing of Specialist only/ initiated medicines (SO/SI). Support prescribers in the use of the formulary tools and local recommendations. Provide education and training to the PCN on therapeutics and medicines optimisation, where appropriate.
