Main duties of the job

The Nurse Associate will work as part of the nursing workforce, supporting the Registered Nurses to deliver high quality, person centred, compassionate care to patients.

The Nurse Associate will work as part of the MDT delivering planned care to a group of patients, working within the scope of the Nurse Associate Role.Working effectively as part of the MDT, they will provide support/guidance to less experienced staff/learners within the clinical team as needed and delegate care if appropriate.Supervise and support care assistant, volunteers and students as appropriate

Job responsibilities

To deliver and monitor care under the leadership of a Registered Nurse, ensuring a person-centered approach.

Form a professional judgement of an individuals condition and report any changes or concerns to the Registered Nurse/medical team.

Complete risk assessment tools appropriately and report any concerns to the Registered Nurse.

Be responsible for a group of allocated patients, including coordinating the patient pathway from referral to discharge, leading on all aspects of goal planning and organizing and leading any team reviews or family meetings, and being responsible for all aspects of patient documentation.

Appropriately refer to members of the MDT, informing the Registered NurseAssist the Registered Nurse to deliver clinical care to patients, working within the scope of the Associate Nurse remit.

Take responsibility for the coaching/training of care assistants and where appropriate, volunteers and students.

Co-ordinate a team of care assistants and where appropriate, volunteers and studentsUndertake the ongoing supervision of an identified group, such as care assistants and/or volunteersUndertake a range of tasks delegated by the Registered NurseProvide concise handovers to the MDT as appropriateMonitor and care for patients receiving oxygen therapy as directed by the Registered Nurse

Develop and maintain competency to undertake clinical skills to enhance the delivery of patient care, such as measuring, monitoring and recording of patient observations, reporting adverse signs to the Registered Nurse/medical team

Perform and record blood glucose levels using the correct equipmentPerform and record urine testing as per organizational policy, reporting and adverse signs to the Registered Nurse/medical team

Assist in the administration of non-controlled drugs once deemed competent to do so.

Perform and record clinical observations including blood pressure, temperature, respirations, pulse.

Recognise issues relating to safeguarding vulnerable adults and report any problems or raise concerns to the Registered Nurse.

Engage in supervision & reflective practice, promoting opportunities for increased self-awareness and learning.

To be second recorder for controlled drugs.Undertake various nursing skills once completed agreed Nurse Associate competencies

To maintain accurate healthcare records, written and computer based, liaising with community colleagues and in line with organisational policy and professional requirements.

To support and work with the nursing to develop and maintain effective communication links with members of the multi-disciplinary team.

Person Specification

Qualifications

Essential

Registered Nurse Associate on the NMC register. Nursing Associate Level 5 Foundation degree. GCSE Grade A-C in Maths & English or skills level 2 Maths & English or equivalent.

Experience

Essential

Experience of working within palliative and end of life care. Experience of working in teams under appropriate supervision as part of the multi-disciplinary.
