Job summary

Harptree & Cameley Surgeries are expanding their workforce - an opportunity has arisen to join our friendly team as a Practice Nurse in a rural community setting.

We are seeking an experienced Practice Nurseable to offer a general practice nurse service to include actively supportingthe practice management team by reviewing treatment room clinical policies andprocedures. Working as a member of thepractice multidisciplinary team, the post holder will ensure nursing servicesare delivered effectively to our patient population.

Hours of work - Ideally 2/3 days per week (Mon/Tues/Wed), hours by negotiation.

Main duties of the job

The practice nurse will be responsible for orinvolved in a number of clinical areas such as infection prevention andcontrol, health promotion, chronic disease management care, well woman and wellman clinics, cytology, wound care, immunisation and other core general practicenurse duties. Experience of asthma care, COPD reviews and coils would beparticularly welcomed.

Job responsibilities

Thefollowing are the core responsibilities of the Practice Nurse. There may be, onoccasion, a requirement to carry out other tasks. This will be dependent uponfactors such as workload and staffing levels:

To supportthe Infection Prevention Control (IPC) lead and to work to the IPC Policy supportingthe implementation of audits, training, support and evidence of compliance tothe practice manager To supportthe production of the IPC annual statement in conjunction with the practicemanager Todevelop, implement and embed an effective care management programme for thefrail and housebound patients registered at the practice Toassess the needs of patients ensuring the provision of healthcare isappropriate, incorporating evidence-based practice Todevelop, implement and embed health promotion and wellbeing programmes Toimplement and evaluate individual treatment plans for chronic disease patientsthat promote health and wellbeing Toidentify, manage and support patients at risk of developing long-termconditions, preventing adverse effects to patients health Asrequired, to provide routine nursing care to patients in accordance withclinical based evidence, NICE and the NSF Asrequired, to diagnose and manage acute and chronic conditions Toprescribe medication for patients in accordance with PGDs and own scope ofpractice Toreview patient medications to enhance compliance To provide guidance to patients in the use of prescribed and over-the-countermedications regarding side effects and dosages Toco-ordinate the provision of travel medicine services Torequest pathology services and process pathology results as required Tooversee the provision of chronic disease clinics, delivering patient care asnecessary, referring patients to secondary/specialist care as required Tomaintain accurate clinical records in conjunction with extant legislation Toensure read codes are used effectively by all members of the nursing team Toensure the appropriate maintenance and use of chronic disease registers Todevelop, implement, embed and participate in well woman clinics Todevelop, implement, embed and participate in well man clinics Toensure compliance with the practice chaperone policy Toassist GPs with minor surgery when required Toprioritise health issues and intervene appropriately Tosupport the team in dealing with clinical emergencies Torecognise, assess and refer patients presenting with mental health needs Toimplement vaccination programmes for adults and children To support patients in the use of theirprescribed medicines or over-the-counter medicines (within own scope ofpractice) To liaise with external services/agenciesto ensure the patient is supported appropriately (vulnerable patients etc.) To delegate clinical responsibilitiesappropriately (ensuring safe practice and the task is within the scope ofpractice of the individual) To support the clinical team with allsafeguarding matters, in accordance with local and national policies To understand practice and localpolicies for substance abuse and addictive behaviour, referring patientsappropriately Todeliver opportunistic health promotion where appropriate

Inaddition to the primary responsibilities, the Practice Nurse may be requestedto:

Developand review audit protocols and process for the practice, effectively utilisingthe audit cycle

Develop,review and implement nursing protocols in conjunction with the partners Supportall members of the nursing team, providing guidance when necessary, conductingappraisals and training needs analyses as required Participatein local initiatives to enhance service delivery and patient care Supportand participate in shared learning within the practice in order to improvepatient care Continuallyreview clinical practices, responding to national policies and initiativeswhere appropriate Participatein the review of significant and near-miss events applying a structuredapproach, , root cause analysis (RCA) Coordinatethe nursing team rota, ensuring sufficient staff are available to meet patientneeds Leadthe planning and implementation of changes within the nursing team, enhancingthe provision of services to patient Support the practice team with the reviewing andimplementation of practice policies and protocols, ensuring conformance toextant legislation Person Specification
