PCMH Clinical Lead

Post holders will have a high level of autonomy working within primary care settings, working within the parameters of the Primary Care mental Health Team standard operational policy Please see Job description for full details of main responsibilities.

The Adult Community Mental Health Teams are multi-disciplinary teams that offer assessment, treatment and care to adults who experience complex mental health difficulties.This is a leadership role and the post holder will work closely with the Team Manager to achieve positive outcomes in the teams' clinical management
