Based at Whittle Surgery, the post is offered on a part time basis of 30 hours per week.

Main duties of the job

This is a varied role and the postholder will work as part of a multidisciplinary team to support and promote safe,rational, high quality and cost effective use of medicines. Within the role you will also support andencourage a safe, responsible and respectful attitude to prescribing andmedicines use within the practice

The postholderwill provide pharmaceutical technical support to the Clinical pharmacist tohelp and support the implementation of practice prescribing initiatives andmedicines optimisation programmes. In addition, they will also support theimplementation of robust systems to facilitate accurate and timely monitoringand reporting and delivery of performance monitoring targets and indicatorsrelated to medicine optimisation initiatives.

About us

Formed in May 22, Chorley Together PCN consists of six GP practices across the Chorley and Leyland locality and serves a population of approximately 38000.

Whittle Surgery is located in modern purpose built premises on the A6 at Whittle-le-Woods and has a current list size of 11500 patients. You will work closely with GPs, Social Prescribers, Mental Health Practitioner, First Contact Practitioner, Care Co-ordinators and practice nursing and admin teams as part of a multi-disciplinary team to deliver a holistic and patient centred approach to care.

Job responsibilities

Key Duties and Responsibilities:

Qualityof prescribing

Support the practice pharmacists withMedication review for all patients on repeat medication and maintenance of themedication review system.

Review list of medicines for newlyregistered patients and add to their medication record as appropriate andensure patients are entered into any monitoring systems, medication reviewdates added.

Ensure annual monitoring tests areperformed and reviewed where appropriate, , ACE-I/A2RA, amiodarone,digoxin, lipid lowering drugs, warfarin, antipsychotics, antiepileptics,antidepressants.

Provide medicines reconciliationfollowing discharge from hospital. Review discharge information and updatepatients repeat medication list as necessary.

Support the practice to improveprescribing quality and performance against national and local targets.

Support the provision of prescribingadvice to prescribers and clinical staff in the practice regarding changes indrug safety information, discontinued products, changes in licensing etc.

Cost-effective use of medicines

Support the implementation of QIPPswitches that have been agreed.

Support the practice to achieve QOF.Involvement in QOF areas as requested by the practice where medications areconcerned

Support patients with good MedicinesUse, , inhaler technique, use of glucometers.

Implement and support RepeatDispensing.

Liaise with local communitypharmacists and act as a point of contact (alongside our Medicines ManagementCo-ordinator) regarding any prescription queries, amendments, or appropriateswitches for out-of-stock medicines.

Liaise with local care homesregarding medication requests and queries.

Act as a point of contact forprescribers and pharmacies for patients using monitored dosage systems andrepeat dispensing relating to any changes in drug treatment.

Review out-patient letters whichinclude changes to drug treatment and make changes to patients repeatmedication list as appropriate (or query prescribing if not in accordance withFormulary)

Review out-patient dischargerecommendations and issue prescriptions as requested.

Report any secondary carerecommendations to prescribe non-formulary or red drugs to the local MM team.

Support queries from reception staffregarding patients making unusual requests for past medication, overuse ofmedication and refer to GP as necessary.

Assist with prescribing auditsarising from safety concerns or good prescribing practice.

MedicinesOptimisation

Provide information and guidance topatients to improve adherence with medicines taking.

Promote and support the use of shareddecision making.

Monitor use of drugs for patients whomay be overusing or misusing drugs and alert prescribers as necessary.

Respond to queries from GPs and otherclinical staff and contact patients where necessary.

Respond to queries from receptionistsregarding any medicine-related matters and contact patients where necessary.

Develop communication systems toensure effective dissemination of information within the practice.

Ability to handle sensitive and confidential data Excellent communication and interpersonal skills, (both written and verbal) including dealing with patients and carers Ability to plan and work independently and manage competing priorities Ongoing commitment to CPD to maintain theoretical and practical knowledge Motivated Strong IT skills, particularly in Microsoft products Ability to prepare and present reports. Qualifications

Essential

Registered Technician with the General Pharmaceutical Council Recognised qualification in pharmaceutical sciences (BTEC/NVQ3 or equivalent) plus specialist training and experience to degree level or equivalent
