Job overview

We are looking for an enthusiastic, motivated senior clinical pharmacist to work closely with the community-based staff and be responsible for the provision and continued development of a clinical pharmacy service for the Wigan and Leigh Specialist Community Mental Health Team (SCMHT) including an Outpatient Medication Review clinic. 

You will link closely with both inpatient teams and community teams ensuring a seamless care approach. The post holder will co-operate in ensuring the provision of high-quality patient focused pharmaceutical care.

Main duties of the job

You will be responsible for running pharmacy services to the Community Mental Health Teams . You will work under the supervision of senior pharmacists providing a clinical pharmacy services, running a medication review clinic and setting up innovative ways of working.

The role will include daily team visits, supporting the multi-disciplinary by attending care planning meetings and line management/supervision of pharmacists , technicians and the wider team. 

You will be supported by members of the leadership team including a team leader, and clinical lead. CPD opportunities are available and personal interests/skills will be supported to develop and grow.

Detailed job description and main responsibilities

To attend MDTs for each team on a regular basis. To prioritise patients with complex medication needs for medication histories, medication reviews and clinical input. To provide pharmaceutical care to the Recovery patients citywide providing a clinical pharmacy and prescription monitoring service ensuring safe and appropriate use of medicines in order to maximise benefit and minimise risk to patients. To provide a prescribing clinic for recovery patients when fully qualified. To act as an expert source of pharmaceutical advice for health and social care staff, patients and carers. To provide a clinical pharmaceutical opinion when medical information is conflicting.

Education & Qualifications

Essential criteria

Pharmacist registered with the General Pharmaceutical Council. Post graduate diploma or master’s qualification in clinical pharmacy or able to demonstrate equivalent knowledge through relevant experience.

Desirable criteria

Non- Medical Prescribing Qualification and be a Registered Delegated Prescribing Practitioner

Experience

Essential criteria

Experience of delivering a clinical pharmacy service including prescribing Experience of working in mental health. Evidence of sound technical pharmaceutical & clinical knowledge to optimise safe and effective use of medicines. Experience of policy/procedure writing and implementation Experience in cost improvement programmes/managing changes

Desirable criteria

Conflict resolution experience

Knowledge & Skills

Essential criteria

Excellent communication skills verbally and in writing Able to plan own workload with priorities and objectives and cope with conflicting pressures Able to work as a team and independently Ability to influence the prescribing practice of others. Evidence of involvement in education and training

Knowledge of current issues in community pharmacy
