We are a popular, growing town practice in Newbury, Berkshire who is looking for a Clinical Pharmacist to join our team. The successful applicant will work with our existing Pharmacist.

Newbury is a desirable market town, surrounded by lovely countryside, good rail links and excellent local schools.

Main duties of the job

We are looking for a full or part time Clinical Pharmacist, to provide specialist pharmaceutical support to to improve and support safe and effective prescribing.

Day to day tasks may include medication reviews, reconciliation of hospital discharge summaries, managing prescription requests, involvement in MDT meetings and implement change with MHRA alerts.

We are a friendly and supportive practice, offering a happy working environment. Our clinical team consists of 10 GPs, 2 Paramedics, 2 Pharmacists, 1 ANP, 4 Practice Nurses, 2 HCA's and 1 Matron.

We have a team of 10 Receptionists, 4 Care Coordinators, 5 Secretaries, 2 Administrators and 3 Team Managers.

Job Summary

Thepost holder will work within their clinical competencies to provide specialistpharmaceutical support, including with working with members of the widerhealthcare team to improve care and support safe and effective prescribing.

Therole will have a patient facing element either face to face or telephone -including chronic disease management, repeat prescription management and structured medication reviews. This willalso include the management and review of patients with polypharmacy.

Thepost holder will need to be proactively involved in quality improvement andaudit as well as managing some aspects of the Quality and Outcomes Framework.

Therewill also be the requirement for the post holder to contribute to the provisionof out of hours/extended hours/improved access services.

Thepost holder will be supported to help develop the role and work to improve thecare and outcomes for the patients across the PCN.

Toplay a pivotal role in the delivery of high quality primary health care withinthe practice.

Job Summary

Clinicalmedication reviews for patients with single or multiple long term conditionswhere medicines optimisation is required

Toreconcile medicines following discharge from hospitals or intermediate care andworking with patients and community pharmacists to ensure patients receive themedicines they need post-discharge and to reduce risk of readmission.

Tobe involved in multidisciplinary clinical meetings within the Practice and PCNwhere appropriate

Contributeto the provision of out of hours/on call/extended services for the practice andthe patients. These can include patient facing and telephone consultations,signposting to other services and/or healthcare professionals whereappropriate, while working within scope of practice and limits of competence.

Tomanage repeat prescription requests, resolving queries where possible withinscope of practice. To ensure patients have appropriate monitoring tests in placewhen required.

Implementchanges to medicines in line with MHRA alerts, product withdrawal or shortageand other local or national guidance

Tomaintain full and complete records of all patient contacts using appropriateclinical templates and coding

Toundertake a proactive role in audit and quality improvement implementingrecommendations where appropriate.

Provideindependent information to patients with regard to medicines and prescribingchanges, initiating further support from other healthcare professionals whereappropriate

Participatein review and setting of policies relating to medicines management andprescribing and help ensure practices prescribe in accordance with localguidelines and formulary

Toidentify areas of clinical risk and make recommendations to support theintroduction of new working practices that will optimise the quality ofprescribing and make more efficient use of network resources.

Tomonitor network practices prescribing against the local health economy andmake recommendations for GPs or amend within scope of practice, based onfindings.

Tomaintain own clinical and professional competence and be responsible for owncontinuous professional development

Person Specification

Qualifications

Essential

Professional Registration with General Pharmaceutical Council.

Desirable

GPhC Independent Prescriber Qualification is desirable.

Experience

Essential

Minimum of two years working as a pharmacist

Desirable

Experience in managing pharmacy services in primary care Broad knowledge of General Practice
